Dear New Grad DPTs! , Many recent DPT graduates and students are doing internships in private clinics without having completed a house job or training in teaching hospitals. Unfortunately, many institutes lack affiliation with teaching hospitals, preventing them from providing standard supervised clinical practice and house jobs. My advice to them is that while it’s fine to work as an intern in clinics, you will only observe or treat a limited range of cases in msk, neuro, or pediatric areas. It is essential to work in a teaching hospital to gain exposure in different wards and units, including pre and post-operative rehab, critical care physiotherapy in ICU and CCU, and dealing with complex patients, such as those with metastasis. You will learn to treat patients ranging from neonatal to geriatric while collaborating with other health professionals. Remember, your opportunities are not limited to small clinics, your job could include from emergency care in the ICU to working with field athletes. After gaining this exposure, you can decide on your future specialty, whether it be sports and musculoskeletal physical therapy, strength and conditioning, neuro, critical care physiotherapy, pediatrics, pelvic floor physical therapy, and others. Keep in mind that physiotherapy isn't just applying modalities like electrical stimulation, heat, ultrasound, and manual therapy in a private practice.

Small steps are the right beginning - well done

The problem is not with the idea of a third medical school itself. The problem is that we are losing doctors faster than we can train them. In the hospital we are already struggling to train junior doctors and medical students due to high workload, poor staffing. I’m sure you’ve heard it all before. It’s great to want to produce more GPs, especially rural ones. But as it stands, I am hearing from medical students and GPs alike, that it is very challenging to find placements for students. It’s these placements, which are like a brief apprenticeship, that confer the best learning opportunities and insights into what a career in general practice can look like. These placements are what shapes future doctors. So a new brick and mortar medical school by itself will do nothing to fix the issues of lack of investment in medical training and staffing. Doctors from all walks of life are crying out for more support and more staffing. Primary care is on its knees and if you think a 6 hour ED wait time is bad, it will only get worse if we continue to underfund primary care. This is how you build a strong #health system and make #primarycare an easy choice for #medical graduates. I love what #Europe does for its citizens in #healthcare. That said, as a #healthtech #entrepreneur, I learned quickly that to build an early stage #venture, I needed to have focus on one customer type in one system, low #legal and #compliance costs, and fast path to scaling. So, I needed to build in the U.S., not try trans-Atlantic partnerships too early. As it is, the U.S. is already very fragmented and complicated, with multiple payers and stakeholders. Really would love to have this access for myself and to work in a system like this. Imagine my health and lifespan would improve and extend.

Volunteering at Hospitals or Clinics What it offers: Students can gain firsthand exposure to patient care and the healthcare environment by volunteering at hospitals, clinics, or nursing homes. This experience is highly regarded by medical schools. Opportunities: Help in various departments such as pediatrics, emergency room, or physical therapy. Some programs offer administrative roles, while others involve direct patient interaction. 2. Medical Internships/Shadowing Physicians What it offers: Shadowing physicians allows students to observe doctors' daily routines, patient interactions, and medical procedures. Medical internships provide more in-depth hands-on experience in a clinical or research setting. Opportunities: Many hospitals and private clinics offer summer internships for undergraduates. Some universities also have partnerships with local healthcare providers to facilitate shadowing programs. 3. Health-Related Clubs What it offers: Joining health and medical clubs allows students to engage with peers interested in healthcare careers, participate in healthcare-related discussions, and work on health-focused community projects. Examples: Pre-med societies, public health clubs, global health clubs, or student-run health organizations. 4. Community Health Outreach What it offers: Participating in health outreach programs or initiatives helps students improve public health through education, vaccination drives, or health screenings. This activity also demonstrates a commitment to serving the community. Opportunities: Many universities have partnerships with local health organizations or host health fairs where students can volunteer. 5. Scientific Research What it offers: Research in biology, chemistry, or health-related fields helps students develop critical thinking skills and contributes to their understanding of medical science. It's also important for students interested in careers in medical research or academia. Opportunities: Students can work in university labs, participate in summer research programs, or assist professors with ongoing research projects.

There is?new federal funding dedicated to providing #CNAs (Certified Nursing Assistant) with meaningful opportunities for career advancement: Back in November, the Health Resources and Services Administration (HRSAgov), HHS posted a Notice of Funding Opportunity (NOFO) for its next cohort of Geriatric Workforce Enhancement Programs (GWEP). Stick with us here... The NOFO specifically requires that GWEPs "using the apprenticeship framework, provide education and training for direct care workers to support career advancement (ex. Certificate, etc.) within their discipline as geriatrics specialists." The requirement goes on to specifically call out "the development of career ladders for certified nursing assistants to become advanced geriatrics specialists." This is #progress in the right direction... And, exactly in alignment with our Action Plan: "Expanding Certified Nursing Assistant (CNA) Career Pathways". How is that >98% of Danes have a GP? Part of the answer lies in their approach to training physicians. Here's a summary of some of what I've learned on my trip to Denmark ???? After high school, most Danes take a gap year where they typically work and travel (the average is 2 gap years but several take even more). They would then apply and gain admission to medical school which is 6 years in total. After graduating from medical school, every new doctor does a one year "internship" of sorts, 6 months of which is in a GP practice and the other 6 months in a hospital. Where they do this internship is based on luck. They draw numbers and choose the location based on numerical order. If you draw 10, you pretty much have your pick of spots. If you're 450, you'll have pretty limited choices left. Following the internship, they apply and get selected into an orientation for a specialty program. This orientation is 6 months for GPs (remember they already did 6 months in their internship) and 1 year for other specialties. It's a time for residents to assess whether the specialty is the right fit for them and vice versa. When they complete the orientation, residents can apply for an orientation to another specialty area if they choose. On average, most residents do two orientations. When they are sure which specialty they want to pursue, they apply and are selected to a residency program. Residency training is 4.5 years for GPs and 5 years for other specialties. In the GP residency, trainees spend 30 months on hospital rotations and a 6 month, 6 month and 12 month block in 3 different general practices. Notably, the GP residency increased from 3.5 years to 5 years in 2003. I've been told that this change actually made it easier to recruit trainees to become GPs and that following the change, GP incomes increased. GPs now, on average, make about the same or more than colleagues in other specialties. There are a few interesting features of the physician training pathway in Denmark: -all specialists gain a foundation in family medicine through the internship year -the location of the internship year is based on a lottery, exposing trainees to communities across the country and likely increasing the chances of them settling in a more rural area -trainees have a chance to try out different specialties with the orientation year -GP training is about the same length as other specialty training I think elements of this training pathway are part of the reason why general practice is highly respected in Denmark and paid as well as other specialties. Pay and respect in turn influence recruitment and retention of the GP workforce. PS. the photo here is a wall of some of the 170+ residents trained at the practice I visited (more on that later!)
