?????? ?????????? ???????????????? ???????? ???? ???????? ?????????????? ?????????????????? 3?? 2024 The CMS Hospital at Home (HaH) program, initiated during the COVID-19 pandemic, has revolutionized healthcare delivery by enabling hospitals to provide acute-level care in patients' homes while receiving Medicare reimbursements. This program has rapidly expanded across 365 CMS Certification Numbers (CCNs) in 138 health systems in 39 states by Q3 2024 (https://lnkd.in/enj8FrFg). Major health systems such as Mass General Brigham, Mayo Clinic, and Mount Sinai Health System have been instrumental in adopting this model, which has demonstrated the ability to improve patient outcomes and reduce healthcare costs. HaH allows hospitals to offer patient-centered care, where individuals can recover in the comfort of their homes, promoting quicker recovery and lower stress. Technology integration, particularly through Remote Patient Monitoring (RPM) devices, ensures that patients are closely monitored in real time, reducing hospital-acquired infections and the need for physical hospital beds. As the HaH model continues to gain momentum, it is proving its long-term value by offering scalable solutions for expanding care capacity, especially during periods of high demand. However, with the program's waiver set to expire at the end of 2024, the future of HaH remains uncertain. Policymakers are working to extend the waiver for an additional five years to secure the program’s benefits. HaH represents a transformative shift in healthcare by blending cutting-edge technology with patient-centered care, providing a glimpse into the future of flexible, efficient, and cost-effective healthcare delivery.
