Transforming Medi-Cal for Over 14 Million Californians The California Advancing and Innovating Medi-Cal (CalAIM) initiative represents a monumental shift in how our state delivers Medicaid services. Through new programs and reforms, CalAIM strives to: ?? Reduce system fragmentation by integrating managed care plans more fully with county services ??Support whole person care needs with enhanced care management and in Community Supports ??Invest in social determinants of health like housing transition navigation services ??Improve quality outcomes and reduce health disparities through population health management strategies ??With $782 million allocated from the state general fund in 2021-2022 and more investments planned in the coming years, CalAIM brings the potential for better health and well-being for millions of low-income Californians and communities of color. CHCF is closely tracking the CalAIM rollout and supporting safety net providers to successfully transition. The scope of this ambitious initiative is far-reaching, but we believe it puts our state on a path to a more equitable, integrated, and patient-centric Medi-Cal program. https://lnkd.in/gpyAtmAc
