Our friendly clinical admin team plays a crucial role within the Hospice, often serving as the first point of contact for patients and their families. Clinical Admin Manager, Charlotte, said, "We feel fortunate to support patients and those close to them throughout their journey, from initial referrals to coordinating with external clinicians, and organising specialised bereavement support during difficult times. "Earlier this year, the Hospice launched a three-year clinical strategy, and one goal was to promote integrated working. To support this, the admin team introduced a new working model that includes dedicated team members embedded in key clinical areas for Community Outreach, Bereavement and the Inpatient Unit, along with an admin hub. This change enables us to better assist clinicians, allowing them to concentrate on delivering high-quality care. To enhance visibility, the team also decided to adopt uniforms, making it easier for colleagues, patients, and visitors to identify us. This has been well received, contributing to a more collaborative environment and reinforcing our commitment to providing excellent care and support. "By implementing these changes, we aim to strengthen our overall support and foster deeper connections within the #Hospice community." #Healthcare #HealthcareProfessionals #Hastings #Rother #Administration #ClinicalAdministration

For those who know me, you know that HOSPICE will always have a special place in my heart. As we observe National Hospice and Palliative Care Month, I couldn't let this time pass without sharing something close to me. ? CHOICE MATTERS. (Read that again.) ? Over the years, I’ve often heard colleagues, physicians, social workers, and referral sources say, “We let the patient choose.” I wholeheartedly agree. But here's the critical point: Don’t take away your patient’s ability to choose by avoiding the hard conversation. If you choose to avoid the tough talk, you’re making the choice for them. Patient choice starts with education. It’s our job to have those difficult conversations, to share the full picture, and to give patients the knowledge they need to make decisions aligned with their values and preferences. Time and time again, the most common thing I hear from families I’ve had the privilege to serve is, “We only wish someone would have told us about hospice sooner.” So, let’s take the leap. Let’s be the ones who step in, have the tough conversation, and offer the option of hospice care. Let’s give our patients and their families the chance to make an informed choice. Talk about hospice. Be the hero in their journey. #HospiceCare #PatientChoice #PalliativeCare #HealthcareProfessionals #EndOfLifeCare #EducationMatters #NationalHospiceMonth Lindsey Ingle Natalie Skinner Joshua Bennett Denise Lalka Dawn Mullins Bryan Pottow, LNHAAlicia Patten Williams Brandii Niles Kristen Paro Metoda Posega ,RN Julie Couden Eileen Orel Heather E. Finding the best care for your loved one can be exhausting. Our goal is to make the process easier and less stressful. Here are some valuable tips for finding the right senior care options: 1. Conduct Thorough Community Reviews: When researching senior living options, ensure you conduct detailed inspections. Meet with the staff, engage with residents, and review state inspection reports to confirm the quality of care. 2. Explore a Variety of Care Choices: From Independent Living to Assisted Living, Memory Care, and Skilled Nursing, explore the different types of care available to find the right fit for your loved one’s unique needs. 3. Seek Customized Support: Consider arranging community tours, asking comprehensive questions, and understanding the paperwork and negotiations involved. Having a clear roadmap can guide you every step of the way. 4. Understand the Costs and Services: Be aware that some placement services may be complimentary, as they are paid by the care communities. However, if a community is not part of a network, there may be potential fees, so it's essential to ask upfront. We aim to help you simplify your search for the best care, taking the stress out of finding the perfect choice for your loved one. ?? #SeniorCare #AssistedLiving #SeniorLiving #ElderlyCare #CareOptions #ExpertGuidance

An opportunistic "loophole" in NY PHL 2805-x has sparked outrage among hospice advocates. The convenient language was recently used to circumvent the CON process, threatening the existence of several long-serving, quality-driven hospices that serve their communities. The result? Potential poaching of staff and closed system referrals that will likely result in a decrease in the census for already existing hospices. No, wait…it gets even better. The collaboration was only discovered thanks to a social media post. The legislature never intended for the law to be used as a means to avoid the CON process, and without amendment deprives existing facilities of due process, deprives health care consumers of services aligned with community need, creates health care disparities, and exacerbates issues involving social determinants of health that the CON process was designed to prevent. We are calling on lawmakers to REJECT language found in sections 1-3 of Part V of the Health & Mental Hygiene Article VII bill, and require stakeholder input by the Executive, DOH & provider advocates on CON reform as part of a larger effort after the budget is finalized. We are asking the legislature to INCLUDE language that would prohibit the recently expanded interpretation of the PHL 2805-X waiver which effectively subverts the CON process and its intent to preserve health equity. The law needs to align with the State Administrative Procedure Act to require public notification and opportunity to comment when an application is made for a regulatory waiver, and to ensure that any such application meet applicable CON requirements. As New York State considers changes to its current CON process - and as holes in the state budget are filled - it's important to ensure that the quality-driven hospices are protected and that the CON process is not exploited. #hospice #CON #healthequity #nys

Fundamentally throughout our lives, we all want to be treated with respect and dignity. It’s essential that people being looked after in care homes feel understood and supported, and have an active role in their own care. The Care Quality Commission (CQC) define dignity and respect as “making sure that people have privacy when they need and want it, treating them as equals and providing any support they might need to be autonomous, independent and involved in their local community.” Whether it’s through personalised treatment plans, facilitating independence or supporting your patients’ needs, how do you promote dignity in care?
