Congrats to our own Dr. Lynn Yee (MACA board member and co-medical director of our hotline) on being one of the principal investigators for a new study that will attempt to answer the question: How does a pregnant person’s environment, diet, stress, medications and social wellbeing affect their pregnancy and — down the road — their child’s health? That will be the focus of a new two-year study from scientists at Northwestern University - The Feinberg School of Medicine and Ann & Robert H. Lurie Children's Hospital of Chicago, which will examine how environmental factors affect children, including those with a variety of disabilities. The scientists recently were awarded $5 million from the National Institutes of Health (NIH) to join a national consortium — the Environmental influences on Child Health Outcomes (ECHO) Program — that will examine how perinatal and early childhood environments and experiences influence the health of children as they grow and develop. While other awardees will focus on asthma and cardiometabolic health in children, the scientists at Northwestern and Lurie Children’s will specifically focus on people with disabilities who have historically been left out of medical research. “ECHO is a huge national cohort of children’s health, and with this study, we’re finally studying their mothers, too,” said Northwestern co-principal investigator Dr. Lynn Yee, the Thomas J. Watkins Memorial Professor of Obstetrics and Gynecology at Feinberg. “There's so much to be learned about the mother-child dyad and the bidirectional ways in which they influence each other." It's not lost on me that when my daughters become pregnant they may face much greater challenges than I did a few decades ago. When I was pregnant with my first daughter, I was a health policy analyst at the American Academy of Pediatrics knowing way too much about my risks, including my chance of having a low birthweight baby given the research on Black mothers at the time. As if that was enough to worry about, now, the risks are greater. New research from my team at NORC at the University of Chicago highlights the need to address unequal maternal and postpartum outcomes. American mothers experience severe complications at a high rate; among them, Black mothers experience much higher rates of these near-death events than others, during childbirth and after. It will be a while before my daughters become mothers, so I hope that research from my team and others will lead to significant improvements. As research professionals, we have a responsibility to utilize available data to identify root causes of complications during and following birth. Within this, calling out inequities is only the first step – we must continue to probe to find addressable clinical and social drivers of these outcomes. ?? The Challenges of Waiting for Pediatric Specialist Appointments: A Personal Journey ?? As a parent, navigating the healthcare system can often feel like an odyssey, particularly when it involves specialized care for our children. Currently, I am experiencing this firsthand, as we are on a waiting list for a pediatric psychologist since April 2023. Despite regular check-ins, there is no availability until possibly April 2024. This extended waiting period brings to light the significant challenges many families face in accessing pediatric specialists. It's not just a matter of waiting; it's about the impact this delay has on a child's development and well-being. Early intervention is crucial, especially in mental health, where timely support can make a profound difference in a child's life. The shortage of pediatric specialists is a pressing issue. It's not just about the number of professionals but also about the geographical distribution and accessibility of these services. Many families have to travel long distances, compounding the stress and financial burden. This waiting period can also be challenging for parents. The uncertainty and concern for our child's health can lead to feelings of helplessness and frustration. We are left navigating the complexities of our children's needs without the professional guidance we urgently seek. It's a call to action for healthcare systems to address these gaps and ensure that children receive the timely and specialized care they need. Investing in training more pediatric specialists and improving healthcare infrastructure is vital. To those on similar journeys, I extend my empathy and support. Let's continue to advocate for better healthcare access for our children and share our stories to raise awareness of these critical issues. Thanks to the magic of technology, I was able to listen to ACGME Vice President for Wellbeing Dr. Stuart Slavin give grand rounds at the University of Wisconsin-Madison Department of Pediatrics this morning. This point really resonated with me: We are all co-creators of our culture. Yes, the system is broken. No, we don't have to be miserable while we work to fix it. ?? What are the benefits and harms of proposed social media legislation? Specifically, for minors, do the potential harms outweigh the benefits??? I was reading a viewpoint published in JAMA Pediatrics by Megan A. Moreno, MD, MSEd, MPH, and Jenny S. Radesky, MD, and it has made me really stop and think about this. Are the authors of the legislation and/or the authors of the article overstepping here? I see where there are circumstances or situations where a minor (someone less than 18 years of age here in Florida) can benefit by reaching out to likeminded peers. This is especially true for those who do not have the family (and close friends) support they need or if they physically live in an isolated environment. But are the two opposing sides in this argument overreaching? Of course, I am biased. I am the proud parent of three amazing boys (ranging in age from 6 years to 8 months). I do not want my kindergartener to have his own social media account. My wife and I are very careful with the kids. (Even in my personal blog, I do not use my children's names. I've let them pick their monikers--Spider-Man, Bonecrusher, and A-Bomb.) I think there is a difference between the ages in 6, 13, and 17. There should be different, age-appropriate (or maturity level-appropriate) measures in place. What do you think about this topic? Delving into the World Breastfeeding Awareness Week initiative led by SCOHE and SCOPA, where we meticulously executed FOUR pivotal programs from August 1-7, 2023. At the core of our efforts were; 1.Capacity Building Webinar, titled 'Nurturing Breastfeeding Advocates: Empowering Medical Students for Workplace Support,' providing medical students with crucial resources through infographics and pertinent links. 2. Our advocacy journey extended to university stakeholders, urging the creation of breastfeeding-friendly environments on campuses. 3. Outreach programs echoed the importance of breastfeeding, emphasizing the pivotal role of societal and workplace support for breastfeeding mothers. 4. The Social Media Campaign served as a digital rallying point, amplifying the significance of breastfeeding and workplace support. 5. The creative force was unleashed through the Poster Design and Poetry Contest, engaging over 50 medical students. Their graphics now actively contribute to the narrative, championing breastfeeding-friendly workplaces. The impact has been resounding; over 90% of participants from the webinar now feel adequately informed and equipped to become effective breastfeeding advocates. University student associations have committed to advancing conducive breastfeeding spaces, and the contest graphics are actively shaping perceptions. “Postpartum and postnatal care are essential to new mothers and their babies, yet Black women are half as likely as white women to receive the appropriate care, leading to higher rates of complications.” ? As a Black mother who struggled with getting the kind of care I needed during my pregnancy, I know that diversity, equity, inclusion and compassion in healthcare are critical to solving the Black maternal health crisis. ? ?? Family Reflections: Prematurity is for Life, Not Just the Neonatal Unit ?? In 2006, Mandy Daly's journey into motherhood took an unexpected turn. At just 25 weeks pregnant, complications arose, leading to a life-threatening placenta abruption. Her daughter, Amelia Faith, was born weighing a mere 780 grams. The experience in the Neonatal Intensive Care Unit (NICU) redefined their lives. Key Reflections for the Scientific and Clinical Community: -The Reality of Preterm Birth: Preterm infants are not just smaller; their neurological, sensory, respiratory, and other systems are underdeveloped. This complexity demands a nuanced approach to care. -Parental Challenges: The transition from the NICU to home is daunting. Parents often feel unprepared and overwhelmed by their new caregiving roles. There is a pressing need for better education and support for families post-discharge. -Long-term Health Implications: The journey does not end at NICU discharge. Chronic lung disease, sensory processing disorder, and developmental challenges continue to affect premature infants' daily lives. These experiences highlight the importance of extended follow-up care for preterm infants into adulthood. -Pain Perception and Sensory Issues: Preterm infants undergo numerous painful procedures, impacting their future pain perception. Sensory challenges can affect all aspects of life, from school performance to daily functioning. -The Need for Specialized Care: There is a critical need for the development of sub-specialties to provide ongoing care for adults born preterm. A family-centered and developmentally supportive care model in the NICU is essential to improve long-term outcomes. -Research and Advocacy: Future research must be informed by the lived experiences of preterm infants and their families. This holistic understanding can drive better clinical practices and support systems. Amelia's story is a testament to the resilience of preterm infants and their families. It underscores the necessity for a lifelong, supportive healthcare framework to ensure these children can thrive.
