So great to be in post-production! @NoCountryforOldPeople is needed now more than ever as Congress advances a bill to ban CMS from setting minimum safe staffing standards in nursing homes. This is the product of the millions of dollars in nursing home industry campaign contributions and lobbying (mostly paid with taxpayer dollars, by the way). It ignores the findings of a new study, Tunneling and Hidden Profits in Healthcare (Mar. 4, 2024), that analyzed related party transactions data for Illinois nursing homes (the Feds and nearly all other states don’t require nursing home owners to report related party transactions). Key takeaways: -??????63% of nursing home profits were hidden and tunneled to related parties through inflated transfer prices. -??????industry groups representing providers who rely heavily on public reimbursements plead poverty to advocate for higher reimbursement rates or against costly quality regulations, pointing to public cost reports that indicate artificially low or even negative profits due to tunneling. -??????related party implied markups are substantial. These large markups correspond to staggering hidden profits...estimates suggest that reported nursing home profits reflect only 37.1% of total profits as of 2019. We should all be outraged that Congress is bending the knee to owners who have for years secretly tunneled billions of dollars in profits from their nursing homes (funded mostly by taxpayer dollars), while claiming they can’t afford adequate staffing and need more taxpayer dollars to stay in business. The study highlighted by the HAP Foundation and NORC reveals significant barriers faced by Black communities, in accessing hospice and palliative care. From my own experience as someone with Caribbean heritage, I understand the perception within our community that engaging with hospice care is like admitting defeat—a final surrender, which makes discussing it even harder. This cultural view makes it crucial for healthcare providers to navigate these discussions with great sensitivity and understanding. One of our volunteers, Ed, is a stellar human being and we are very lucky to have him on our team. He used to be a Hospice Social Worker and now just serves others out of his own sense of purpose and we are grateful to have him on this team. The other day he shared a quote from Maya Angelou, "There is no greater agony than bearing an untold story inside you." Ed took the time to remind us some important things and I thought this message was important enough to share here. As hospice workers, we have a unique opportunity to help bring peace to patients and families during the end of life journey. One of the ways we can do this is by helping people tell their story without judgement. We can be there as a listening ear. We can pull in our social worker or chaplain to expand on this listening and even bring in a volunteer. We can take it a step further and help them by recording or writing down their life review. We can help our patients get in contact with family. Truly, sometimes there are unspoken apologies that once said, or even once written down (as that might be easier) can release so much agony in a person. In what ways do you as a healthcare worker help your patients tell their own stories?
