It’s #WorldLungDay, so let’s talk about bronchiolitis and continuous pulse oximetry monitoring—why are guidelines not being followed? Continuous pulse oximetry (cSpO2) monitors contribute to noisy care environments, emit frequent false alarms that can unnecessarily burden nurses, and they elicit excessive intervention. Even so, cSpO2 monitoring outside established guidelines is common in children hospitalized with bronchiolitis. To understand why, Clinical Futures’ core faculty member Halley Ruppel, PhD, RN, and Associated Director of Implementation Science, Chris Bonafide, MD, MSCE, and their research team analyzed 371 clinician responses across 36 hospitals and found that 70% of responses did not include a clinical reason. Last week, at Acute Medicine Days in H?meenlinna, Finland, the Systematic team, Lene Buch and Timo A. Rantanen, showcased the Columna Flow Clinical Tasking mobile solution. The system helps assign patients to doctors and nurses, considering the actual workload and enables smooth and secure communication between the clinicians in various roles at the Emergency Department (ED) ???? The first pilot EDs in Denmark have reduced the number of phone calls interrupting work significantly - by up to 90%. Improving ED workflows through digitalisation is also raising interest among Finnish acute medicine professionals. We look forward to kicking off the pilot project with the first Finnish EDs to get evidence of how the benefits will be realized outside of Denmark. The EMF Effect: Inspiring insights from Dr Michelle Dodds and Natasha Edwards from Redcliffe Hospital Emergency Department on their research into developing a paediatric peripheral intravenous cannulation package. Funded by an EMF Emerge grant, the project aimed to enhance the procedure for inserting intravenous cannulas in children under 16, which often causes anxiety for patients and families. The research team developed evidence-based resources, including a bedside guide and an education package, to boost clinician confidence and standardise practices. The implementation of these resources resulted in improved patient experiences, reduced clinician anxiety, and enhanced workflow efficiency. These resources have been successfully integrated into various departments at Redcliffe Hospital and have also been adopted at many sites in Queensland. Future plans include expanding the resources for broader applications in both paediatric and adult care, focusing on skills that alleviate patient anxiety during various medical procedures. The Maryland Regional Neonatal Transport Program recently welcomed a new member to their team: a brand-new vehicle, specially equipped with the latest technology to serve infants in the region with serious health needs. Johns Hopkins neonatologist Frances Northington, co-medical director of the neonatal transport team, describes the vehicle as a “NICU on wheels,” noting, “Our team can perform medical procedures during transit if necessary.” The transport program has been jointly operated by Johns Hopkins and the University of Maryland Medical Center for the past 30 years.
