As part of my presentation at the ASRM course this year in Denver, Colorado, discussing how to find surgical and non-surgical approaches to address fibroid treatment inequity. I highlighted many examples but this is a classic example of the agony some women in the Middle East suffer due to deep cultural beliefs about preserving virginity. Our patient, a well-educated lady from the Gulf area, fully sponsored by her government, suffered from multiple fibroids, one of which was 7 cm and deeply impacted in the vagina. She experienced severe anemia and took many medications, both hormonal and non-hormonal, for many years. Her BMI increased to 41. She and her family refused any procedure involving a vaginal approach. She traveled for multiple international consultations, but they consistently refused any open or vaginal technique. This case illustrates how cultural and religious beliefs can influence and affect a patient's quality of life. As medical professionals, we must find ways to respect their wishes while improvising to help them. I am not only honored that the American Society for Reproductive Medicine chose me as a pre-congress faculty member, but I also thank them for allowing me to express and spread an important message to all members interested in fibroid treatments. We sat down with Mina Alikani, Giles Palmer, Jacques Cohen, and Francesca Farlie from Alpha Scientists and the International IVF Initiative, lead authors and the organizing team behind recent paper ‘Sustainability in the IVF laboratory: recommendations of an expert panel’, to discuss the genesis of the project, the issues they are tackling and their recommendations for the future of green IVF. One of the areas we get involved in is research into new treatments and tests for pre-eclampsia and this paper out today - the Parrot 2 Trial is essential reading for clinicians and commissioners on the value of repeated PlGF testing. Marcus Green, CEO of Action on Pre-eclampsia, said: “These are really important findings showing that once the first test has been done, there is nothing to be gained from further testing. A single test can assure women with certainty if they are likely or unlikely to get pre-eclampsia.?We look forward to completion of roll-out of placental growth factor-based testing across England, with an urgent call for implementation in the devolved administrations across all four nations. This new study from Raquel Del Gallego, Barbara Lawrenz, Baris Ata, Erkan Kalafat, Laura Melado Vidales, Ibrahim ElKhatib, and Human M. Fatemi,MD,PhD demonstrates the additive value of an almost forgotten and neglected ovarian reserve parameter in prognosticating unexpected poor or suboptimal response especially in women ≥35 years. Really valuable bit of research - earlier work has demonstrated the utility of a predictive test for #preeclampsia (much cheaper and easier than doing placental blood flow analysis from ultrasound, too) - but doing more tests doesn't add further value. It is as important to know when and where #NHS tests do NOT offer benefits as when they do! It may not have the glamour of a 'shiny new thing' (test/product/intervention) but this is evidence that commissioners need...and from which patients and the public benefit.
