Today, we're diving into bone health and the crucial role of biomarkers in managing osteoporosis. Let’s put your diagnostic skills to the test! ?? ?? Today's Question: What role do CTx (C-terminal telopeptide) and NTx (N-terminal telopeptide) play in the management of osteoporosis? ?? A. Increased urinary excretion is diagnostic of early-stage disease B. Increased levels indicate a low risk of developing osteoporosis C. Decreased urinary excretion indicates a positive response to treatment D. The rate of urinary excretion correlates with the stage of the disease ?? Correct Answer: C. Decreased urinary excretion indicates a positive response to treatment! ?? ?? In-Depth Interpretation: CTx and NTx are bone turnover markers used to measure bone resorption (the process by which bone is broken down). These markers are often used to monitor the effectiveness of osteoporosis treatment. We're back with the answer of the first case. Clinical history: A 73-year-old female patient presents to the emergency room with abdominal pain in the flank and right iliac fossa, which started approximately 4 days ago, accompanied by nausea. CT findings: A retroperitoneal collection is projected subhepatically, extending inferiorly along the right parieto-colic gutter, heterogeneous due to the presence of internal gas bubbles. Foci and small gas bubbles extend inferiorly along the ascending colon to the ceco-appendicular region. At this level, the appendix appears at the base with thickened walls, up to 5 mm, possibly indicating a perforated appendix with a subhepatic collection. Surgical findings:?Perforated gangrenous acute appendicitis with generalized purulent peritonitis and appendiceal phlegmon. Histopathological exam: Two fragments of adipose tissue, with a partailly filiform appearance(diameter 3 mm) and partially thickened walls up to 1,3 cm, with fibrin on the surface. The second fragment is adipose tissue with areas of fibrosis and a filiform appendix. Acute appendicitis. Stay tuned for the next cases.

In exudative #AgeRelatedMacularDegeneration (#WetAMD), significant strides — such as the development of anti-vascular endothelial growth factor (#AntiVEGF) therapies — have pushed the treatment possibilities forward and rewritten the likely course of the disease for many patients. However, for most patients, #AMD is nonexudative (dry), and treatments for those patients remain largely limited.1,2 Recent developments in the form of #ComplementInhibitorTreatments (such as pegcetacoplan and avacincaptad pegol) are showing promise in slowing progression rates of #GeographicAtrophy (GA) — an advanced, vision-threatening form of #DryAMD affecting nearly 1 in 5 patients. Our recent study took a closer look at this by having three Reading Centers (GRADE, CIRCLE, M3) review OCT images. The results? In everyday practice, fluid is missed more often than we would like to admit. And even when it’s spotted,?????? ?? ?????????? ???? ?????????? treatment is skipped for diabetic macular oedema or oedema secondary to retinal vein occlusions. What is the reason? Sometimes, it is our own hesitation to act when we are not convinced ourselves. The classic excuse of “stable function” does not really cut it, especially when the potential for further improvement is right there!
