Therapists, in graduate school, we often hear about the importance of professional development, particularly in establishing your "therapeutic or theoretical orientation." And it doesn't stop there—we're also encouraged to focus on this in supervision. It's crucial to understand, as it influences how we view our clients, conceptualize cases, narrow our niche, and even recognize potential biases where fellow therapists' perspectives are invaluable. To help refine yours, here are 3?? questions that help me! 1?? How do you believe change happens? Take your time to slow down and carefully break down your answer. Your response will offer valuable insights into how you view personal growth, psychological change, emotional regulation, and the journey from where someone is in their life to where they want to be. 2?? What do you believe causes anxiety? This question has many layers, so notice where your thoughts lead you. Do you focus on gut health, nutrition, environmental factors, or perhaps someone's thought processes or unconscious mechanisms? Reflect on what you emphasize in your answer and what you may overlook. 3?? What do you believe your role is in therapy, and what contributions do you offer to the therapeutic process? Do you emphasize the relational aspect? Are you providing analysis, structure, and behavioral planning? Do you take a collaborative approach, or do you lean toward a more independent method? For many graduate students, this can feel like a pressure-filled question. I want to reassure you that you don’t need to have it all figured out right now (or ever) because it will evolve over time. That being said, understanding the foundation of where you naturally gravitate as a therapist—in your interests and vision for your practice—can help you work more authentically. Relatedly, if this interests you, I strongly encourage you to read the original works (at least once!) of the founding psychological theories. While textbooks can provide helpful summaries, they often miss the depth, nuances, and underlying philosophies the original authors intended to convey. Lately I have been stimulated by some considerations while reading the latest book translated into Italian by Arthur C. Brooks.? The thesis supported by Brooks, which has been corroborated by many studies, is that there is an important cognitive bias regarding the intellectual performance of so-called white-collar jobs. The popular vulgate would have it that mental performance is the same throughout life and only deteriorates with senility. However, the studies cited above seem to suggest that at best mental performance in these types of jobs peaks around 20 to 25 years after completion of studies and generally never exceeds 50 years. This means that in a work environment the performance of those over this age tends to decline compared to that of younger people. The problem is that given the rather long life expectancy, this means that work-wise we have very experienced people who have about 10 to 15 years of work ahead of them (perhaps in management position) in which they make more mistakes and whose ability to process complex information decreases. As population aging progress in western countries this issue is something enterprise and governements should deal with. Building Professional Confidence: Elevating Our Identity in Physical Therapy Confidence in ourselves and our professional practice is the first step toward elevating the value of physical therapy. Unfortunately, the imposter phenomenon is widespread in our field, with many practitioners struggling with self-doubt. The current professional landscape does little to bolster self-assurance, which can hinder growth and fulfillment. We must focus on strategies to empower practitioners and foster professional development as an industry. Research shows that imposter syndrome is prevalent among practicing PTs, especially early-career professionals, and can lead to burnout, job dissatisfaction, and psychological distress. By acknowledging this challenge, we can work toward creating a supportive environment that prioritizes confidence, self-awareness, and personal value. Our professional future rests in the hands of the new generation of clinicians. What we do to empower growth in the first 5 years of clinical practice will reflect the profession's value for the following 30 years. How often do you place your well-being on the back burner in the rush of meeting professional demands? Dr. Shane T.
