How do I help Families? ~ In SO many ways! I actually starting this series #WhatIDoWednesday in an effort to educate and explain to people what it is that I do. Because when you have created and offer something that is new and different, you need to explain to people exactly what you do. In fact, most of the time when people contact me, they start with, "I'm not sure exactly what you do, I was just told to call you..." Here are a few examples of #whatido: ~ Meet with people to discuss next best steps before their family member comes home from hospital. ~ Work with people to develop a care plan and enhance their #careteam to best support their family member to be safe and well at home. ~ Visit family members of those sons and daughters who live out of province/country but have concern about the health and wellness of their Mom and/or Dad, here in the local area. ~ Collaborate with other professionals, doctors and specialists to make sure that people are getting the services, treatment and equipment that they need.
