More importantly, as a local hospital, it's important to us that we are transparent with our community and share information about our successes and opportunities for improvement. That's why we're making our Critical Access Hospital Annual Report public and why we're sharing it with you. This report includes information about our service volume, type of service, trends, concerns, accomplishments, and more. This report is used by our Leadership Team and Board of Trustees to develop action plans, identify opportunities for improvement, and recognize our successes. It is a sad testament to the state of healthcare in New Yorks State that there are more F rates facilities in New York than anywhere else, and New York does not boast one single A rated facility. I fear the situation will only worsen especially with recent reports that there are more rural hospitals at risk of closure in New York than any other state. As a hospital emergency doctor, Touchstone CEO Merran Cooper spent hours chasing advance care plans for her patients, and was repeatedly provided with documents that contained errors such as boxes ticked for CPR and no CPR. That's because advance care plans – despite being vital documents that clinical teams often need to access urgently – are usually created and stored as paper forms and PDFs. A picture is worth a thousand words. Many states still receive very low scores on the Leapfrog patient safety ratings. Why do we allow healthcare facilities to operate despite these low patient safety scores? Hospitals with a patient safety score of C or lower have an over 80% higher fatality rate due to preventable medical errors. These hospitals are endangering patients, while our regulators seem to look away. "The Leapfrog Group recently contracted with the Johns Hopkins Armstrong Institute for Patient Safety and Quality to update its estimate of deaths due to errors, accidents, injuries and infections at “A”, “B”, “C”, “D” and “F” hospitals.?Overall, an estimated 160,000 lives are lost annually from the avoidable medical errors that are accounted for in the Leapfrog Hospital Safety Grade, a significant improvement from 2016, when researchers estimated 205,000 avoidable deaths. The analysis also found that D and F hospitals carry nearly twice the?risk of mortality of A hospitals.?Over 50,000 lives could be saved if all hospitals performed at the level of A graded hospitals."
