As some of you know I have recently returned from a trek out to the #BigRedBash in Birdsville, Outback Queensland. A bucket list item ticked off for me and an absolutely awesome, be it challenging experience with my loved ones! But besides that, an eye-opening experience in relation to the challenges regional and remote Qld faces in accessing the basic necessities when distance and weather come into play. Although I have been involved in the planning and delivery of health services across Queensland for many years, the reality hit home when only 20mm of rain cut all road access leading 120 strangers to form an unexpected "community" for three days. Where I live, we easily get 100mm plus in an hour in rain events without blinking! Some told stories of being caught for longer on their travels on other roads due to mechanical issues and reliance on parts being able to be flown into remote properties to enable them to carry on their way. How did that resonate to me? How na?ve I was regarding the impact weather has in isolating communities. How planning and delivering health facilities to access care closer to home can quickly come unstuck with only 20mm of rain. How important partnerships are when planning health facilities across #OutbackQueensland to overcome these access issues. The Big Red Bash is a huge supporter of the Royal Flying Doctor Services. Where would our health services be without #RFDS, #QueenslandAmbulance, our other health partners and remote communities all working together. I take my hat off to rural and remote communities and how they cope with these uncertainties. Certainly, this experience and the importance of partnerships and contingencies will remain forefront in my thoughts when planning our future health facilities. #health #planning #infrastructure #partnerships #community

Increased travel, changing climates, and pesticide resistance have led to an alarming surge in bed bugs, hitchhiking into hotels on guests' luggage. Recently, these pests even invaded government offices, affecting the UK Health Security Agency and Medicines and Healthcare Products Regulatory Agency on the fourth floor. Cromwell or Queenstown? The curly question put to me yesterday: Where would you put the Big Base Hospital ? I would have to say. "Cromwell, but then Queenstown - if that had to be" And it seems agreed - not Wanaka. But there is quite a lot to the question for the districts of Central Otago Queenstown Lakes. # The centre of population is somewhere between Roaring Meg and Cromwell, so near Cromwell. Which makes it the shortest commute for the most. # The Wakatipu basin (includes Queenstown) has the most people. # Wanaka Alexandra and Queenstown are on the edge of the area. Cromwell is in the middle. # We are not rural despite vast landscape everywhere you look. We have a population of 80,000 now and 100,000 soon. Compare that to Dunedin with a non growth 130,000. Note all the fuss over a new hospital there. # We need a base hospital. Most (70)% of the people in New Zealand who live more than two and a half hours from a base hospital live here. # Size matters ! with reasonable size there comes a point when it is possible to have an on site anaesthetist overnight. Not to even mention having doctors at all. # There are staff politics to deal with. Because a single base hospital means that Queenstown and Dunstan Hospitals will close. Perhaps better to describe that as “moving”. But uproar is likely. # “Centers of Excellence Idea” Different centers doing different things. Promoted I guess to keep the current small hospitals, but not for the public interest. Without scale the activities in each smaller unit would be limited. # Commuting. Yes we have winter and a couple of roads can be awful some days. You certainly don't want to go directly between Queenstown and Wanaka at those times. So Cromwell. # Commuting 2. Interestingly our commute times are similar to Auckland. Often quicker than Auckland. Just because you see some paddocks it's not time consuming. Many people live in one of our four centers and work in another. 40 minutes between most places. One hour and 10 minutes Wanaka to the Airport.

I agree with many of?Kerry Hand's points, particularly regarding the need for a base hospital in the Queenstown Lakes District Council or Central Otago District Council Region and the importance of considering population growth and commuting times. However, our region's geography and topography make this a very challenging question. QLDC's population predictions indicate Queenstown will remain the largest population centre until at least 2053. They also predict a continued much larger daily visitor population in Queenstown than the rest of the region. We should also factor in?geographic factors?beyond simple distance/time. The region's susceptibility to?natural disasters, such as earthquakes, should be a key consideration. In a significant earthquake,?road closures (The Kawarau Gorge and The Devil's Staircase) would make the region's largest population vulnerable to isolation. Public-access emergency healthcare will be needed in all centres of the region, no matter where a new base hospital is (e.g., Queenstown, Wanaka, and Cromwell/Alexandra). Ultimately, the location of the base hospital should be based on a comprehensive assessment of factors such as population growth, commuting times, geographic risks, and the potential impact on residents and visitors to the region, not simply commuting times.
