Ask WHY? Not Who ? Focussing solely on individuals without understanding cause and context will not improve safety. It just makes it worse. If it’s a genuine mistake, a few questions to consider asking: Why did this make sense at the time? ? What was your perception of it? ? What was this based on? ? What influenced your decisions? ? How can we improve this? ? What learning have you taken from it? ? This takes more time than the alternative ‘traditional’ approach (i.e. a telling off or some form of penalty), but you get all the time back when the underlaying causes are dealt with and you’re not wasting time dealing with the same incidents over and over again. Plus your culture no longer stinks.
