I agree with the very smart and fastidious author: pretty much everyone I have seen commenting on the Cass review is, to some extent, wrong. It’s a bit depressing, really - people who oppose any medical care for transgender individuals have taken up the review as a perfect document with no flaws. At the same time, those who object to the findings seem intent on promoting untrue claims about perceived weaknesses in the report. If you have the time, judge for yourself. This from a sociologist who has been studying the social aspects of transgender medicine in the U.S and globally for the past 7 years. For those of us in the AEC Industry, it's important to recognize that the downstream effects of such legislation (through their varying impacts on building owner decision-making and the exclusion of certain key stakeholder groups during design) can negatively impact our ability to create environments that are inclusive and welcoming. That includes achieving the International Living Future Institute's Just Label (particularly the gender diversity indicator in 2.0, with version 3.0 as I understand it improving the label's accounting of gender identity, including relevant indicators related to belonging and equitable recruitment). In addition, the following point from Dr. Lynne-Joseph about focusing solely on randomized control trial data is important to highlight. "Proponents of bans on gender-affirming health care for people under the age of 18 often argue that there is inadequate scientific evidence for this care. Many of the criticisms center on the lack of randomized control trials (RCTs), which are considered to be the 'gold standard' of evidence in medicine. However, these criticisms overlook that RCTs are not ethically feasible in many areas of medicine (not just gender-affirming health care), and that other forms of evidence can and should be evaluated to assess the risks and benefits of various interventions." I'm delighted that my just-completed article Criminalizing Transgender Care will appear in the Iowa Law Review this fall. Abstract Since 2021, twenty-one states, in extraordinarily quick succession, have enacted statutes banning physicians from prescribing puberty blockers and cross-sex hormones to minors for treatment of gender dysphoria. Although the Food and Drug Administration has not approved these drugs for this use, off-label prescribing is a common practice, and leading medical organizations all agree that this off-label use of puberty blockers and sex hormones is an essential component of transgender medical care. These state laws thus represent an extreme, and unprecedented, interference with the provision of standard-of-care medicine. This article, after exploring the ongoing litigation challenging these bans, argues that they violate a fundamental right under the Due Process Clause of the Fourteenth Amendment—namely, the right to obtain standard-of-care treatment from a physician. It demonstrates that this right is deeply rooted in America’s history and traditions by presenting the first-ever comprehensive review of state policies regarding off-label prescribing practices and showing that the states have virtually never interfered with physicians’ prescribing decisions in this manner. Finally, in light of relevant judicial precedents, this article shows why courts should strike down these unparalleled, oppressive state laws as unconstitutional. https://lnkd.in/eKq2vEXY

Understanding and addressing the healthcare needs of transgender individuals is an ongoing challenge that requires careful consideration. Hormone therapies play a crucial role in the lives of many transgender people, providing a means to live authentically and reduce mental health risks. However, the use of these therapies, especially in younger individuals, remains a topic of debate due to concerns about fertility and irreversible effects. While research is being conducted to better understand the effects and benefits of hormone therapies, there is still much we don't know. The recently released 2022 U.S. Transgender Survey shows that 94% of trans adults living at least partly in their identity are more satisfied with their lives, and 98% of trans adults on hormone replacement therapy (HRT) are more satisfied with their lives. From The Guardian: “Obtaining hormone therapy through the informed consent model and having access to surgery has definitely changed my life for the better,” one survey respondent, named Taylor, said. “It certainly didn’t fix all of my problems instantly, but it fixed a lot of problems that my dysphoria caused. Now that I’ve been on HRT and had surgery, I can live my day-to-day life without pain, dissociation, and misery.” The 2022 survey is the largest ever study of trans adults, with 92,000 participants. Minority populations, including Black and Native American people, people of color, cultural and ethnic minorities, LGBTQ+ people, migrants and refugees, and the socioeconomically marginalized all face serious disparities in pregnancy, childbirth and postnatal health care outcomes. This study sought to understand how ecotherapy for marginalized expecting couples can support attachment repair, foster family resilience to mitigate the stresses of marginalized pregnancy and create a protective benefit for navigating maternal health care. A literature review provides critical synthesis of three areas of discourse, perinatal mental health and maternal health care, attachment theory and attachment-based intervention, and ecopsychology and ecotherapy. An attachment-based ecotherapy intervention is introduced, and recommendations are made for future study. The study aims to stimulate new discourse on how ecotherapy can be incorporated in the field of marriage, couple, and family therapy to offer expanded clinical support for marginalized expecting couples. Healthcare Access: Discrimination in healthcare settings may lead to inadequate care and delayed treatment for epilepsy. 3. Socioeconomic Factors: SGM individuals often face higher rates of poverty and unemployment, affecting their access to healthcare and medication. 4. Intersectionality: Those with multiple marginalized identities may experience compounded health disparities. 5. Mental Health Comorbidities: Higher rates of anxiety and depression in SGM populations can exacerbate neurological conditions. 6. Social Determinants: Education, income, and social support significantly influence health outcomes. Addressing these disparities requires improved data collection, culturally competent healthcare, targeted outreach, and policies that tackle socioeconomic inequalities affecting SGM communities. ??
