Last week, the HSJ released data highlighting trusts with the longest discharge delays post being medically fit. It prompted me to reflect on the importance of enhanced system collaboration. While hospitals can contribute more, we require support from various health partners, social care, and patients with their networks earlier in the pathway. The understanding of this crucial need should translate into actions within consultations and ward rounds and I don’t think we’re there yet. I'm curious to learn about how other healthcare facilities have successfully integrated discharge planning into their daily treatment protocols. #Healthcare #DischargePlanning #SystemCollaboration

How can the US healthcare system potentially improve access and care outcomes, while reducing healthcare spending? A recent study of 1000 clinicians by my McKinsey colleagues and authors found that shifting care from hospital-based settings to alternative sites, such as outpatient centers or patients' homes, could potentially save the US healthcare system $113.8 billion to $147.7 billion annually.
