Summary: This groundbreaking study delves into the risk of venous thromboembolism (VTE) post-orthopaedic surgery, shedding light on crucial insights. Researchers examined patients who experienced VTE within one year after surgery, uncovering significant findings. ?? Key Points: Risk Factors Identified: Obesity, personal/family history of DVT, cardiovascular disease, and lower limb arthroplasty were confirmed as pre-established risk factors. Extended Risk Period: Patients with known risk factors may remain susceptible to VTE for a longer duration than previously thought, even with post-operative thromboprophylaxis. Link to thesis: https://lnkd.in/eMcseKQ9 **??????????????** Because orthopaedic procedures with total joint prosthesis and instrumented spinal fusion have shown good results, with an increase of quality of life, a further increase of these procedures in the coming decades is expected, with more aged patients, more revision procedures, but also more younger patients who will undergo these procedures. Unfortunately, orthopaedic implants have a risk for bacterial infection. In literature the incidence of surgical site infection (SSI) after spinal surgery ranges from 2 to 12%, depending on diagnosis, surgical approach, use of spinal instrumentation, and the complexity of the procedure. Apophyseal injuries are common in youth sports, with recovery times typically ranging from 4 weeks to 2-3 months.?In a recent case study of 22 young athletes with apophyseal injuries, a single session of focused extracorporeal shockwave therapy (f-ESWT) completely eliminated pain in 14 patients (63.3%).?For the remaining 8 patients, complete pain relief was achieved after the second f-ESWT session in 7 patients (31.8%), and after the third session in 1 patient (4.5%).This research demonstrates the efficacy of f-ESWT in treating apophyseal injuries and its pain, as it allowed the majority of athletes to return to their previous activity significantly faster after initiating treatment.?The authors concluded that f-ESWT is a safe and effective treatment for apophyseal injuries that can facilitate an early return to sports. New Study: Gluteal Tendinopathy and Hip Arthroplasty Outcomes https://lnkd.in/eQ9wTq9b https://lnkd.in/ehed3fdc With the outstanding team from last year’s fellowship at the North Sydney Orthopaedic and Sports Medicine Centre’s group, we examined the impact of gluteal tendinopathy on the outcomes of total hip arthroplasty. The study reveals that more severe cases of gluteal tendinopathy are linked to worse patient-reported outcomes one year after surgery. These findings emphasize the need for routine intraoperative assessment of the gluteal tendons to refine prognostic evaluations and patient counseling. PRP is beneficial in specific scenarios and might be more effective than corticosteroids and HA in certain conditions. However, the differences in how PRP is prepared and the protocols for its use will continue to be its downfall in clinical research and patient outcomes. "Among the studies utilizing PRP for treatment of musculoskeletal/orthopedic pathologies, less than 20% provided a clear, well-described, and reproducible PRP preparation protocol, and only one-fourth of studies reported on the final PRP product composition." #prptherapy #prp #orthopedics #pain #injection

Can you answer our FREE Question of the Day? An orthopaedic surgery resident proposes doing a randomized clinical trial comparing open versus arthroscopic treatment for septic arthritis of the native knee. They perform a pilot study with 10 patients in each group and report their early findings using 95% confidence intervals. They found that arthroscopic treatment does not result in a significantly different hospital length of stay compared to open treatment. However, they proceed to include multiple other community hospitals in the same city and continue the study over the next two years with a final 100 patients in each group. Their final report published in a large journal states arthroscopic treatment results in a significantly shorter hospital length of stay compared to open treatment (Figure A) and formal comparative analysis shows a p-value of 0.04. One in 10 hospital deaths in Australia are from blood clots. New research published today in the Internal Medicine Journal suggests that patients who undergo total knee and total hip replacements may not be getting the best treatment to prevent deadly blood clots following surgery. “More than half (58.4 per cent) of our cohort had one or two concurrent risk factors for aspirin resistance, of which, 42.4 per cent received aspirin, which was the most used medicine prescribed to this group to prevent blood clots after their knee or hip replacement" says Dr Nameer van Oosterom, the lead researcher of the study. “We don’t know the degree of increased risk of blood clots that aspirin resistance has for patients having a TKA and THA patients – but if there is any indication based on aspirin resistant patients who have had heart and brain conditions, it’s likely very significant" READ MORE: https://lnkd.in/guNtrDjk
