Delighted to work on this review discussing the role of Lipoprotein(a) reduction in cardiovascular disease published in the Journal of Clinical Medicine. Lipoprotein(a) is six times more atherogenic than LDL cholesterol on a per-particle basis, is largely genetically determined and a risk factor for coronary heart disease and aortic valve stenosis. There are currently no approved drugs for lipoprotein(a) reduction in clinical practice, but a number of clinical trials are ongoing. Pelacarsen is undergoing phase 3 clinical trials (Lp(a) HORIZON) with earlier trial data showing a dose-dependent reduction in lipoprotein(a) between 35-80%.

Very prescient publication. In Scotland we have had a very wet Spring with much growth of grass/heathland etc and finally a spate of good weather to encourage folks to roam the beautiful countryside and there are indeed some parallels with the NE corner of the USA. The two important take home points are that a tick ideally focusses upon a secure position within the skin layers for at least 48 hrs before thinking of feeding. Standing your kids up on the kitchen bench and doing a front and back tick check, after bath and before bed is ideal. The second and important point is that I note that even many of my well informed patients still refer to whether or not they see a typical bulls eye rash (erythema migrans) but this may only be present in no more than 30% cases. Any associated rash can take numerous forms and it is also commonplace to see some transient redness around the site of tick-skin penetration which should quickly fade after removal. If the rash IS seen then it is very meaningful but if it is not then do not (potentially) falsely reassure yourself. On a separate note we would ideally have access to a more robust confirmatory test and the current position is unsatisfactory in terms of how long it takes for early IgM antibodies being (potentially) detected in a small serum sample and a parallel public concept of LD testing being unreliable and there being “fringe” laboratories (previously in Florida and mainland Europe) who would allegedly report a positive on every expensive sample sent to them. It is also important to fall back on your fundamental skills in terms of clinical assessment and to be aware of the possibility (not inevitability) of any neurological features and not to hesitate discussing with the local hospital and considering the option of a lumbar puncture. Delayed diagnosis of neurological LD is not where anyone wants or deserves to be and it is invariably far more problematic than grappling with the likes of Long Covid. There are some parallels but they are threads of similarity. Good piece of reading. And timely reminders.
