A virtual wards operational framework has been published by NHS England, designed to help support consistency and the achievement of goals set out in the urgent and emergency care recovery plan and 2024/25 priorities and operational planning guidance around maintaining a virtual ward capacity of over 80 percent. The framework details a vision of What Good Looks Like for virtual wards, as well as scaling virtual wards, assessing progress, building an evidence base, recommended indicators and outcomes, through to operational and implementation requirements for integrated care boards. Mass General Brigham has withdrawn its physicians from Steward Health Care's Holy Family Hospital campuses in Haverhill and Methuen, Massachusetts. ? The decision, communicated via email to staff, cites concerns about patient safety standards at the hospital. Steward Health Care responded, expressing regret and emphasizing the hospital's positive safety rankings. ? The current government has made cutting NHS waiting lists one of its key priorities, while one of the Labour party’s five national missions is to ‘get the NHS back on its feet’, including by cutting waiting times. In this IFS pre-election briefing, we outline what has happened to NHS waiting lists (in England, given that health is a devolved responsibility) over the last 17 years – the period for which consistent data are available – and present new scenarios of what could happen to waiting lists over the years to come. Together, with Allina Health, we are solving the challenges surrounding patient throughput. In the last year, working together, have saved nearly 25,000 inpatient days, reduced discharge to skilled nursing facilities by 1.61 days, and decreased discharge to home health by a length of .89 days. How are ICBs tackling delayed hospital discharge and improving patient flow? In a feature for Healthcare Leader Mark Creelman, Merton and Wandsworth Place Lead, describes how virtual wards are providing patients with the right care at home. "Across South West London, there are four virtual ward – hospital at home – services covering our six boroughs, all with face to face and remote monitoring capabilities to provide hospital level care for patients who would otherwise be on a ward, in the comfort of their own home where it is safe to do so. "We now have 415 “beds” with a central remote monitoring hub operating 24 hours a day, 7 days a week to support patients’ own monitoring, and we continue developing our virtual wards to improve services, help ease pressures on urgent and emergency care and contribute to reducing avoidable admissions." Care is moving home... USC Schaeffer Center study published in JAMA detailing the benefits of hospital at home. Benefits include: Patient Preference: Many patients prefer receiving care at home due to the comfort and familiarity of their environment, which can positively impact their recovery experience. Safety and Confidence: The majority of surveyed individuals feel safe managing medications and care at home, indicating trust in home-based healthcare services. Faster Recovery: A significant portion of respondents believe that recovery is quicker at home, potentially reducing the overall burden on healthcare facilities. Cost Efficiency: Hospital-at-home care can lower costs by reducing the need for hospital stays, which often include higher overhead and resource expenses. Policy Support: Temporary programs allowing hospital-at-home care during the pandemic have been extended, reflecting positive policy momentum and recognition of its benefits. I have personally experienced long, excruciating wait times at local community hospitals when my mother was still with us, and I can attest that this significantly impacts patient satisfaction. While data shows that Veterans tend to be more satisfied with the care they receive at the VA, it’s possible that they may not be fully aware of the range of services available at their local VA Medical Center for them to utilize! ****How ER Wait Times Compare: VA Medical Centers vs. Community Hospitals**** 1. Introduction: Emergency room wait times are a significant metric when it comes to patient satisfaction and overall healthcare delivery efficiency. Below is a comparative analysis of wait times at community hospitals and VA Medical Centers, illustrating where each stands in terms of providing timely care. 2. Community Hospitals (CMS Data): ?Based on CMS data, the longest average ER visit times at certain community hospitals can reach up to 9 hours from arrival to departure. Example of the top 3 longest times: ?Hospital A: 556 minutes (~9.3 hours) ?Hospital B: 503 minutes (~8.4 hours) ?Hospital C: 496 minutes (~8.3 hours) Reference: Community hospital wait times obtained from Becker’s Hospital Review. 3. VA Medical Centers (Data Snapshot): ?Data from a VA Medical Center shows that patients in this system experience a considerably shorter wait time compared to some community hospitals. ?Average Admit Delay: 199 minutes (~3.3 hours) ?Percentage of patients boarded for >4 hours: 35.3% ?Patients admitted in under 4 hours: 64.7% Additional details from the data show trends by: ?Day of the Week: Longest delays on Sundays and Mondays. ?Gender and Age: Most patients are between 51 and 64 years old, with males being the predominant group. Note: The data highlights that while delays exist, VA centers tend to manage patient flow more efficiently in terms of minimizing extensive delays compared to the longest waiting community hospitals. 4. Conclusion: While ER wait times at both VA Medical Centers and community hospitals can vary depending on several factors, the VA appears to maintain shorter average times compared to the community hospitals listed in the CMS data.
