Piper Alpha Disaster Late in the evening of 06 July 1988, a series of explosions ripped through the Piper Alpha platform in the North Sea. Engulfed in fire, over the next few hours most of the oil rig topside modules collapsed into the sea.? 167 worker died and many more were injured. This accident the world’s biggest offshore oil disaster.? What went wrong on Piper Alpha?? Why did it have such disastrous consequences? And what lessons can still be learned today? It wasn’t easy to find the cause of the disaster. Little physical evidence remained, and no senior member of Piper Alpha’s management team survived. The day of the accident, condensate pump was isolated for maintenance on its motor drive coupling. Pump pressure relief valve had also been removed for maintenance under a separate permit and a blind flange fitted in its place. The flange was not, however, leak tested or pressure tested.(This mistake came to disaster),(For temporary assembles, take full precautions)? When second pump tripped, the operators tried unsuccessfully to restart it. It is believed that the operators took steps to reinstate pump and condensate leaked from the blind flange which had been installed in place of the pressure relief valve, but not fully tightened up.(Flammable liquid spread rapidly.) The escaping condensate ignited. The first explosion was quickly followed by an oil pipe rupture and fire. The sequential failure of the gas lines then caused a rapid escalation of the disaster. What have we learned from this accident? Many lessons can be drawn from the tragic events on Piper Alpha; this share focuses on six key points... 1. Permit to work and isolation for maintenance (pump re-started before maintenance complete, poor maintenance safety). 2. Handover (inadequate transfer of information between crews, shifts and disciplines); 3. Emergency response, evacuation;(should be tested with training) 4. ?? Project Update: Halfway River Pipeline Bridge ?? We have a significant update on our project with Arc Resources at the Halfway River Pipeline Bridge. Our team has successfully navigated some steep slopes and challenging terrain to ensure safety and efficiency for workers involved in the dismantling process. ?? Accessing the Pipeline: To address the steep and hazardous slope, we were able to utilize the pipeline equipment for worker access. This allowed our crew to safely navigate the slope and perform the necessary tasks, avoiding the need to transport every piece of material down the stairs. This solution greatly improved the efficiency and safety of our team in this remote location. ?? Dismantling Progress: We are also sharing some pictures of the dismantling process. The ability to demobilize material with pipeline equipment, rather than carrying it manually, was a major advantage in this steep and difficult terrain. ?? Some deadly hazards that we don’t often talk about (Part 1) This hazard I’m about to share led to the tragic death of 4 divers while trying to carry out an underwater pipeline maintenance; the removal of an inflatable plug in the process of carrying out the maintenance caused the divers to be forcefully drawn into the pipeline. Paria Diving Tragedy 2022. So what is the mystery around this event? It’s called Delta P hazard: differential pressure, which occurs when two areas separated by a barrier have significantly different pressure levels. In the context of diving, confined spaces, or underwater operations,?Delta P hazards?arise when water, gas, or other fluids move rapidly from a high-pressure area to a low-pressure area through an opening or barrier. This rapid flow can create a powerful suction force that is extremely dangerous for divers, workers, or equipment near the opening.Delta P operations include pipeline or valve maintenance, hydroelectric dams, underwater structures, oil and gas operations, etc. Dangers of Delta P: Entrapment? Crushing or Injury? Asphyxiation? Loss of control A lot of competent and experienced professionals have died due to the effect of delta P hazards in the workplace; therefore, it is important to prioritise safety controls through such high-risk operations by: 1. Ensuring adequate risk assessment before commencing such operation to identify potential areas of delta P occurrence. 2. Use engineering solutions to gradually equalise pressure differences where necessary.
