Transcatheter aortic valve replacement (#TAVR) with a supra-annular, self-expanding valve (SEV) was noninferior to TAVR with an intra-annular, balloon-expandable valve (BEV) with regard to clinical outcomes, and superior concerning bioprosthetic valve dysfunction, through 12 months in patients with symptomatic, severe aortic stenosis who have small aortic annuli, the SMART Trial shows. #ACC24

One of the fastest pulmonary vein isolation using cryoablation worldwide. A 63 year old Patient (195 cm, 120 kg) with symptomstic paroxysmal atrial fibrillation. The total duration of the procedure from puncture of the groin to removal of the sheath was 31 minutes 38 seconds. Fluoroscopy time was only 3 minutes and 26 seconds. Contrast agent volume was 10 ml. Each vein is isolated early with one therapy delivery. Isolation in all PVv is documented. The right upper pulmonary vein showed a short phase of atrial fibrillation before isolation, the dose area product (DAP, cGycm2) was only 334. 28 mm cryoballon from medtronic Rhein-Maas hospital in Würselen- Germany.

Transcatheter or surgical aortic valve implantation: 10-year outcomes of the NOTION trial. Conclusions : In patients with severe AS and lower surgical risk randomized to TAVI or SAVR, the risk of major clinical outcomes was not different 10 years after treatment. Angiographic-derived functional assessment (eg. QFR) has demonstrated a high accuracy in determining the functional relevance of coronary stenoses. It is also useful in planning PCI, utilizing the virtual stenting tool. Here’s a recent case of a middle-aged male with dyspnoea. Resting echocardiogram showed moderate LV dysfunction (EF40%), with LAD territory RWMA. After commencing medical therapy, a coronary angiogram showed moderate-severe LAD disease. QFR which was done within 1 minute with no invasive wire or adenosine confirmed functional significance, with 2 lesions requiring attention. Successfully treated with a hybrid DES/DCB approach using IVUS guidance. Post PCI QFR very reassuring at 0.94.

More big news from the #ESCCongress: In complex PCI cases, OCT-guidance significantly improves clinical outcomes and enhances safety and effectiveness of coronary stenting compared to angiography, as seen in the OCCUPI Trial. Key points to remember: 1?? OCT-guided PCI reduced composite of cardiac death, myocardial infarction (MI), stent thrombosis (ST) & ischemia-driven target vessel revascularization (ID-TVR) by 38% in complex lesions at 1 year compared to angiography. 2?? OCT-guided group patients were 64% less likely to experience a spontaneous MI or require target-vessel revascularization than the angio-guided group. 3?? There was no difference in incidence of contrast-induced nephropathy after PCI between the 2 groups.
