Due to ongoing transition activities under the new administration and new directives for the Department of Health and Human Services (HHS), FDA employees are unable to attend external meetings at this time. Unfortunately, this directive includes our upcoming EL-PFDD meeting on Wednesday, January 29, 2025. The second half of the consultation in PLAB 2 (and the second domain you’re marked on) is management. One of the biggest mistakes doctors make is to just manage the diagnosis and not the whole situation that the patient has disclosed to you in the first half of the consultation ( that’s if you’ve done the data gathering correctly - see my previous post) So they may have told you something that is going on in their life, or something they are worried about, something to do with their lifestyle that could be contributing to their condition. If you ignore these factors and just go straight into the guidelines for managing the condition, say asthma, you won’t be managing the whole situation and ultimately you will lose marks. Even though some of the factors you may not be able to do anything about, you must acknowledge them in the management stage to show the examiner you are trying to manage the complete picture that the patient has presented to you and not just the main medical issue.

Wow!! The numbers continue to increase as we see in FDA and TJC reports related to sterilization and high level disinfection for endoscopes. The root cause of the data is multifactorial. Often the pressure to push for higher volume and revenue from procedures becomes the focus with a negative outcome of sentinal events. The proactive approach is to make sure the procedural area can reprocess endoscopes safely with all the proper tools and supplies, IFU is being followed per each scope model, staff are trained and competent to understand every critical step in reprocessing endoscopes and prevent cross-contamination. Identifying repair needs and frequent maintenance of your endoscopes are critical to reuse the medical device safely. We have a lot of work to get these numbers down!
Here we go—the numbers for Q3: FDA posted 8,719 endoscope-related adverse event reports this quarter, and my team classified 5,279 as relevant to patient safety—sterile processing breaches, residual contamination, retained debris or accessories, infections, or injuries caused by defective scopes. That brings us to over 34,000 in the past year! ? Colonoscopes were at the top of the heap this time, with more than 3,000 reports! There were thousands of adverse events related to gastroscopes and bronchoscopes, too. Oddly, 17% of the reports were missing the event description and narrative, and we hope the manufacturers update those blank documents. ? In the world of youth sports, being diagnosed with asthma of any kind serves as a massive roadblock. As someone who has lived through this as a teen, I understand how living with this condition can hinder a child's ability to develop as an athlete and more importantly, prevent them from playing the sports they love. It's great to hear that the FDA has approved this expanded indication for children with severe asthma, and it's an even better thing to know that companies like AstraZeneca are working towards improving the quality of life of youth athletes with this condition. #FDAapproval #severeasthma #pediatrics

I am so sorry to hear this. ?? Now is the time to sieze the spotlight and make the rare voice heard. The Angelman community has worked hard and I am sure invested heavily to plan this event. Hopefully, the herky-jerky transition will be over soon. There is real work to be done.
