There has been a lot in the media recently regarding ACL management. This is a well rounded report by the ABC that addresses both lay interprettion as well as comment from researchers and specialists. As like most things it at this stage very much comes down to the individual. Our Physiotherapsits are versed in this area and are more than qualified to help you make a decision if you find yourself with a ruptured ACL. Very atypical appointment request this week. We received an appointment request from a prospective patient listing multiple, diverse, lower extremity diagnoses, accompanied by a specific surgical procedure request. This particular procedure did not seem to align with the provided diagnoses in the appointment request we received online. The situation became more complex when the prospective patient contacted my staff by phone, expressing interest in a surgical procedure I discussed in a case report published nearly a decade ago (which was different from the original request of procedure in the online appointment request). This procedure also had no application to the suggested diagnoses. This experience highlights the importance of registering a "complaint" and allowing for an appropriate work-up, including history, exam, and imaging to see what the doctor or surgeon suggests. It’s equally important for patients to be educated and well-versed in any recommended procedure. In this case, my staff could not guarantee that I would perform the procedure mentioned in the old case report. Despite offering to see the patient to offer an unbiased opinion on the condition, it was refused, because I couldn't guarantee that I would perform an endoscopic superior peroneal retinaculum repair for a tibial varum deformity. ??♂?
