?Barbara IOFB Retained intraocular foreign body The luckiest guy! Today’s Eyecare Clinic (in Brescia Italy) Case of the Month was submitted by Barbara Parolini, Veronika Matello, Giulia Freschi, and Roberta Penzani. - This 23YO male was referred for a second opinion regarding a traumatic intraocular foreign body that occurred about 5 months earlier. The patient said that he was pumping a big truck wheel when the tube detached from the wheel’s valve. Metal powder then shot up from the ground and hit the eye. The initial surgical repair involved solely suturing the anterior wound. The foreign body was likely metallic but of unknown type. The patient was asking why the foreign body had been left. Vision was 20/20. - Fundus photography shows a nodular fibrotic scar in the temporal midperiphery. Widefield OCT shows inner retinal hyperreflectivity with posterior shadowing from this lesion. - Siderosis can develop from metallic intraocular foreign bodies (IOFBs, specifically iron) and cause cataract, glaucoma, iris defects, and retinal degeneration with depressed ERG. In this case, we felt no need to remove the IOFB given the encasement by scar tissue, lack of inflammation, and excellent vision. US Olympic Field Hockey player Karlie Kisha was diagnosed with papillary thyroid cancer earlier this year. A week after surgery, she was on a plane to train with her teammates.. Two weeks before the Paris Olympics she was officially in remission. "These are going to be some of the highest highs and the lowest lows that I'll ever go through...It's all part of the process and a part of the journey and I'm just grateful that I'm still able to do this." Five Years Ago, Our Lives Changed Forever: A Journey of Strength and Resilience 5 years ago today, my husband went into hospital for what we thought was an emergency appendectomy. In the early hours after his surgery, I vividly remember asking him if he was now minus an appendix—only to be met with absolute shock when he told me he still had his appendix but had a colostomy bag instead! The doctors had found a cancerous tumour in his small intestine, which had burst, spreading microscopic cancer cells throughout his body. In an instant, our lives changed forever! I’ll never forget the surgeon, Dion Steer, known for his unconventional approach, he was the Doc on duty that night. Had it been anyone else, they might have closed Layne up after seeing what was inside him. But thanks to Dr. Steer's decisive action, Layne had a fighting chance. At the time, Layne was the fittest he had been in years, having recently lost 20kg through proper nutrition and exercise. In fact, Dion Steer mentioned that if Layne hadn’t been in such good shape, the outcome might have been very different. This day marks a life-changing moment in our journey, and I’m forever grateful for the resilience, strength and courage both from Layne and the medical team that saved his life. It’s a reminder that even in the darkest moments, there is hope. With determination, love, and the support of the right people, we can overcome the unimaginable.
