Big news! ?? Congress has announced a $100M increase for Alzheimer’s and dementia research funding at the National Institutes of Health (NIH) for fiscal year 2024! This includes $34M to fund the BOLD Infrastructure for Alzheimer’s Act, which strengthens the Alzheimer’s public health infrastructure in communities across the nation. Over the past 50 + years the movement has become more and more significant and quite frankly, critical. This movement is like no other. It’s about courage, fortitude, perseverance, and compassion. It’s not a job it’s a way of life. It’s about advocating for all. It’s about being a solution for the ills of the world. It’s about lifting up while strengthening America’s landscape. With just a little federal funding, every dime is stretched and generating 6 times over. To kick off National Health Center Week, August 4-10, the White House issued a proclamation recognizing the key role Community Health Centers play in making sure health care is a right in this country, not a privilege: "Federally funded community health centers are a lifeline for over 30 million Americans nationwide — providing affordable access to quality medical, dental, and behavioral health services, which save lives. Community health center sites are especially critical for people in Tribal, rural, and underserved communities because they are often the closest and most convenient sources of care. During National Health Center Week, we recognize the importance of the nearly 1,400 federally funded community health centers that provide critical services to Americans. We express our gratitude to their dedicated staff, who tirelessly devote themselves to the well-being of their patients. Partners Community Health (PCH) is excited to announce the launch of Mississauga Health Seniors Navigation Portal, a ‘one-stop-shop' digital system that will help connect older adults to the services they need or access the support of a care navigator who can assist them. In collaboration with Mississauga Health (Mississauga Ontario Health Team), Caredove, Careteam Technologies, Central Registry and Trillium Health Partners, this navigation portal will help older adults continue to live well at home by connecting them to health care providers in Mississauga. While the Humana Community Navigator (HCN) appears to be a great idea, time will tell how effective this tool is in delivering the anticipated results and achieving a broad level of usage and acceptance. From my experience with providing resource navigation tools for community usage, we can not take for granted that the people who can benefit most from these tools have the ability or the resources to access them. This is particularly true if your target market is Medicare Advantage subscribers. Yes, everyone owns a phone; but not everyone is capable of navigating the ins and outs of apps and web-sites. So what’s the solution? Well, one solution might be to incorporate pharmacies and other frequently visited community locations into the program that have trained employees who can assist consumers in accessing the information and services they require. There are other high-touch points within the community (libraries?) that can be leveraged as well; to the benefits of the MA subscriber and the community as a whole. It certainly seems like a good idea, but there are important guardrails and enhancements that need to become a part of any program like the Humana program. A more complete and targeted program would include some type of Consumer Needs Compass. Such a front end to the Community Navigation tool would allow consumers, providers, members and other constituents to create a path with the most pertinent data SPECIFIC to a person’s individual needs. Lots of data and lots of options are only useful to a point. What all users need to know is the most relevant info for them specifically with some alternatives.
We believe the path to better health shouldn't be hard to travel. To help our members achieve their best possible health, we've developed tools like the Humana Community Navigator? (HCN). The HCN is an active directory that connects members with trusted resources, based on their ZIP code. Thanks to the HCN, all Humana Medicare Advantage members and many Medicaid members can find help with everything from access to food and transportation to job placement services, financial education and much more. Best of all, all the programs listed on the HCN are offered for free, at a reduced rate, or on a sliding scale. Since its launch in 2022, the HCN has made more than 173,000 referrals to more than 51,700 unique members, making it one of the most robust tools we have for supporting our members in their quest for better health and more meaningful community.
