Equip Human Solutions is a partnering up a growing Home Health organization in Salinas, CA! We're seeking experienced PTs to provide personalized care to patients in San Benito and Monterey Counties, Santa Cruz County as needed.

Responsibilities:

• Develop and implement personalized physical therapy plans.
• Assess patient progress and adjust treatment plans accordingly.
• Educate patients and caregivers on home exercise programs.
• Collaborate with other healthcare providers to ensure optimal patient care.

Qualifications:

• Bachelor's or Master's degree in Physical Therapy
• California Physical Therapy license
• Minimum 1 year of experience in physical therapy
• Home health experience preferred
• Excellent communication and organizational skills
• Current CPR certification
• Valid driver's license and reliable vehicle
