Job Description

JOB TITLE: LVN Visit Nurse

DEPARTMENT: Clinical Services

Position Summary

Assists with planning, organizing and coordinating crisis care process, and provides and coordinates nursing care.

WORKING RELATIONSHIPS

REPORTS TO: Team Leader

SUPERVISES: none

INTERPERSONAL RELATIONSHIPS: All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.

EQUIPMENT/MACHINERY USED

Office equipment, automobile, infusion pump, enteral feed pump, CADD pump, nebulizer, oxygen concentrator and tank, suction equipment.

Minimum Training, Education And Experience Requirements

  • Unrestricted license as a Vocational Nurse by the Texas Board of Nurse Examiners.
  • Valid Texas driver’s license, auto liability insurance and safe driving record required.
  • CPR certification.

PREFERRED QUALIFICATIONS

  • 1 year LVN experience.
  • IV Certified.

DUTIES AND RESPONSIBILITIES

  • Provides and coordinates nursing care.
  • Performs focused assessments, i.e. Vital signs, pain assessment, breath sounds, etc. to contribute to the comprehensive assessment by the RN, supporting ongoing data collection.
  • Triages patient care problems
  • Assist the RN or physician in performing specialized nursing skills related to palliative and end-of-life care.
  • Manages discomfort and provides symptom relief in accordance with the Texas LVN Practice Act and their training (See policy H2-058 & H2-059).
  • Initiate appropriate preventive and rehabilitative nursing procedures with scope of practice.
  • Provide education to patient and caregivers re: disease process, death and dying, comfort measures, medication use and side effects, etc.
  • Assist RN in carrying out POC and provides input re changes to the POC
  • Performs ongoing evaluation of pt/family response to care.
  • Assist RN with consultation and educating patient and caregivers (including home health staff) re: disease process, self care techniques, end of life care, death and dying, comfort measures, medicating use and side effects, etc.
  • Assist RN in ongoing assessment of ability of caregiver to meet patient’s needs throughout care and communicate needs/concerns to the IDT.
  • Provides psychosocial support to patients and families
  • Coordinates care with interdisciplinary team
  • Documents in a timely manner patient assessments, patient contacts, physician orders and progress notes that demonstrate progress towards established goals.
  • Assists with planning, organizing and coordinating of care. (10%)
  • Coordinates care with the IDT.
  • Coordinates personal care, DME, and obtaining medications and medical supplies.
  • Informs the physician and other personnel of changes in the patients’ needs and outcomes of interventions as needed.
  • Acts as a liaison between patient and physicians, hospital staff, home health staff, and other care team members to facilitate ongoing clinical care.
  • Documents in a timely manner patient assessments, patient contacts, physician orders and progress notes that demonstrate progress towards established goals.
  • Provides preparation and support for patient’s death.
  • Evaluates own needs for support & self-care and using identified system(s) to meet the need.
  • Recognizes and maintains professional boundaries in relationships with patient/family/caregiver.
