Hospice RN Case Manager

Job Description

Under physician’s orders, the Hospice nurse is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the terminally ill patient/family unit. The Hospice nurse utilizes assessment, intervention and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit. Hospice nurse reports to Patient/Family Care Coordinator/Clinical Director

Qualifications/Experience

1. Current Georgia license as Registered Nurse
2. BSN or Graduate Diploma or Associate Degree Program
3. Minimum of 2 to 10 years nursing experience
4. Minimum one (1) year experience in caring for terminal patients and others with life threatening illnesses.
5. Minimum two (2) years of experience in caring for terminal patients and others with life threatening illness (preferred)
6. Must have reliable transportation, which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must be able to provide proof of automobile insurance, a copy of which will be placed in their employee file

Essential Tasks, Duties, and Responsibilities

1. Provides ongoing assessments of physical, psychological, social and spiritual needs of the patient/family/PCG and initiates appropriate intervention or referral.
2. Provides accurate and appropriate documentation of patient/family services as provided for in the Hospice Care Inc. policies and submits in a timely manner.
3. Actively participates on the Interdisciplinary Team
4. Serves as the case manager to provide continuity of care and submits the required documentation in a timely manner.
5. Assists with providing bereavement counseling to the family and/or primary care giver as determined by the Interdisciplinary Team
6. Evaluates services provided by the HHA/CNA (Home Health Aide/Certified Nurse Assistant) every two weeks with periodic on-site supervisory visits.
7. Demonstrates knowledge of Hospice Care, Inc. policies and procedures and pertinent State and Federal Rules and Regulations.
8. Participates in the team responsibility of providing 24/hour a day, 7 days a week patient care.
9. Recognizes, reinforces and enhances the role of all disciplines in the IDT.
10. Educates the community regarding Hospice Care philosophy and concept through a case consultation with persons in the community or educational in-service or seminar.
11. Represents Hospice Care at nursing functions in the community.
12. Participates in providing education to IDT members regarding nursing aspects of Patient/Family Care
13. Administers medications and treatments prescribed by the attending physician.
14. Attends Hospice in-service and educational programs
15. Weekly review and update of care plans
16. Performs other duties as requested by Patient/Family Care Coordinator

Physical Requirements:

This position requires moving/lifting a bed bound patient who is unable to assist his/her own care in the privacy of his/her home. Weight may vary from 10-250+ pounds.
