NHS cancer care is at a tipping point. Our Lancet Oncology commission highlights 10 key pressure points in our current system and proposes several high value low cost solutions to support the new government to address these time critical issues in the short and long term. However it will require a detailed cancer plan and acknowledgement that excellence in cancer care relies on systems and not on individual technologies or therapies. We need to invest in workforce and capital infrastructure but also research that will help us to deliver the excellent treatments we already have in a timely, equitable and efficient manner. Only then will we start to accelerate improvements in cancer outcomes and patient experience. Thanks to our expert clinical and academic co-authors and institutions. CQC Single Assessment Framework Review by Professor Sir Mike Richards is published today, alongside the final report by Dr Penny Dash. https://lnkd.in/erRzz_xs The Care Quality Commission’s board commissioned this review, which examines the CQC's transformation programme: organisational restructure, single assessment framework, new IT system. Sir Mike’s diagnosis is that the CQC needs a fundamental reset. Key findings 1. Transformation initiatives failed to deliver intended benefits: - CQC struggling to fulfill primary purpose - Fewer inspections, delayed reports - Providers concerned about process and quality 2. Negative staff impacts: - Demoralisation and high turnover - Loss of clinical leadership in inspections 3. Single assessment framework issues: - Too complex, ignores service differences - Confusing elements, delays in reporting 4. IT platform problems: - Adverse impact on staff and providers - Unresolved issues 5. Staffing and morale: - Low morale, insufficient staffing - Inadequate training for new staff 6. Partnership loss: - Providers report diminished collaboration with CQC 7. Limited data usage progress: - Modest improvements in data-informed assessments Key recommendations 1. Organisational reset: - Revert to previous structure - Reinstate sector-based teams led by Chief Inspectors 2. Modify assessment framework: - Retain some aspects, modify others - Suspend evidence categories and scoring system 3. Simplify IT platform 4. Improve data usage: - Incorporate existing datasets - Establish new data sharing agreements 5. Address staffing: - Review levels and pay scales - Fill shortages in key areas 6. Reprioritise inspections: - Focus on high-risk areas given current staffing 7. Rebuild provider partnerships: - Collaborate on assessment design - Consider reintroducing peer review Conclusion Comprehensive inspections and ratings are achievable with adequate staffing, training, and methodology. Urgent action is needed to restore the CQC's effectiveness in regulating health and social care services. The Homecare Association contributed to this review and is keen to ensure our system of regulation is fit for purpose. We want to see a clear plan and swift action to address the many problems created by CQC’s transformation programme. A very helpful summary of where things stand on the CQC Single Assessment Framework. Good intentions, bad execution, compounded problems, limited input from the frontline. We’ve seen it all too many times before - but here’s hoping this report can soon turn from words to action. Our sector needs it.

Have your say on NICE - National Institute for Health and Care Excellence & NHS England’s Budget Impact Test (BIT) threshold for technology reimbursement!? An important consultation on the “Budget Impact Test threshold” is currently open, closing at midnight on the 25th of September 2024. https://lnkd.in/evEm7zNA Irrespective of a technology being cost-effective, the total cost to the NHS also needs to be considered to ensure their budget is not exceeded. To support this, the Budget Impact Threshold (BIT) was introduced in 2017, capping the total budget of introducing a new technology at ￡20 million in any of the first 3 years of a technology’s use in the NHS. If the threshold is breached, NHS England can engage in commercial discussions with the company producing the technology to try to mitigate the impact. If these commercial discussions are not successful, NHS England can then request a variation to the funding requirement from NICE. The current consultation is seeking views on the proposal to increase the BIT from ￡20 million to ￡40 million, following commitments made in the 2024 Voluntary Scheme on Branded Medicines Pricing, Access and Growth (VPAG). This increase, if accepted, would only be applied to technologies that have not yet reached final draft guidance in NICE’s evaluation processes. With the increasing number of innovative technologies and combination therapies being appraised by NICE, an increase in the BIT may allow for new technologies to be recommended, and more importantly help patients in need. However, it is also important to be mindful that the NHS budget is finite, and increasing spending in one indication may impact spending in another. This is a careful balance, and the consultation is an important step in allowing respondents to input into this.? Pelvic health is underserved both in terms of NHS support for patients and in terms of research. Over one-third of women experience long-term postpartum conditions which impact their well-being and quality of life, though many do not seek help. Many women do not seek medical help for postpartum issues due to embarrassment, cultural barriers or lack of time juggling childcare and other commitments. MUTU System is an online programme incorporating pelvic floor exercise videos, education and community. It has emerged as a transformative solution for women's health, particularly in managing conditions such as pelvic organ prolapse (POP), urinary incontinence (UI), and dyspareunia (painful sex). MUTU can be used in the privacy of the home and has videos showing women from different backgrounds, so helps address some of the barriers to access that exist in the current pelvic health pathway. Health Innovation Kent Surrey Sussex commissioned our evaluation partners, Unity Insights, to look at the real-world implementation of MUTU within the NHS. The evaluation showed statistically significant improvements in pelvic health symptoms, positive patient feedback and a potential return on investment over a five-year period within the Kent and Medway Integrated Care System.
