Flap Error: Fatal LAPA 3142 Aeroparque Crash On August 31, 1999, LAPA Flight 3142, operated by Líneas Aéreas Privadas Argentinas, met a catastrophic end when it overshot the runway at Aeroparque Jorge Newbery in Buenos Aires. The Boeing 737-204C, carrying 100 souls, failed to ascend, striking road construction machinery and igniting upon impact, resulting in 65 fatalities and numerous injuries. This disaster, notable for its magnitude within Argentine aviation history, was precipitated by a critical misconfiguration of the aircraft's flaps, set to a retracted position rather than the necessary deployment for takeoff. This oversight was highlighted by the activation of the Take-Off Warning System (TOWS), which the cockpit crew disregarded, leading to the overrun. Investigations revealed a lapse in cockpit discipline and oversight by the operating airline. The aircraft, with substantial operational history and crew experience, showcased lapses in adherence to standard operating procedures. Maintain sterile Cockpit and please follow checklists always. Pay good attention to your ‘Attention Getters’, they are there for a reason. If something is not right and the systems are warning you , do thorough analysis and its consequences. Most importantly study your systems thoroughly. Only then will you be able to make a good decision and avoid embarrassing situations.
