Another public health emergency of international concern, affecting some African countries. While mobilising international partners for a robust response, we shall ensure a more sustainable approach in building stronger disease detection and surveillance capabilities, infrastructures and, human resources. A fire fighting approach has shown limitations learning from crises over time. Many countries are lacking experts for epidemiological analytics including modelling and multisectoral analysis, for treatment of people suffering from contagious diseases (health workers, protocols), for health logistics (to build treatment centers or ensure specific supply chain…), for infection prevention and control among others. It takes for these specialised experts a longer process beyond short trainings and briefings. It also takes the review of curriculum in medical and paramedical institutions to face major local public health risks. Also a longer term vision allowing training of researchers and technical experts to discover new medicines or other countermeasures and ensure a manufacturing capacity for local solutions. We can collectively make it working to narrow the gap between #preparedness and effective #readiness.

The World Health Organization has declared the recent surge of #mpox cases, primarily concentrated in Africa, a "public health emergency of international concern," marking the seventh time the organization has issued such an alert. Alarmingly, children under 18 comprise 70% of cases in the Democratic Republic of Congo, a statistic experts attribute to the discontinuation of the smallpox vaccine (which offered cross-protection) and widespread malnutrition, which weakens immune responses. Dr. Jean Kaseya, Director-General of Africa CDC, expressed grave concern, stating, "This one is a major alarm for the world. We are losing the youth in Africa." The situation has sparked a call to action from African leaders and health organizations, urging a different approach from the global community compared to the COVID-19 pandemic, where Africa felt abandoned. Dr. Kaseya urged international partners to work closely with Africa CDC to provide essential support, including vaccines and diagnostic resources. While some vaccine doses have been pledged by countries like the U.S. and Japan, the current supply falls drastically short of the estimated 10 million doses needed. The high cost of the vaccine – approximately $100 per dose – presents a significant barrier for many African nations already grappling with limited healthcare budgets. The Africa Public Health Foundation (APHF), the fundraising arm of Africa CDC, has launched an appeal for $4 billion to combat the mpox outbreak, aiming to bolster medical treatment, containment efforts, and public health education. As the world watches, the mpox crisis in Africa serves as a stark reminder of the interconnectedness of global health and the critical need for equitable access to resources and support. The international community’s response will be crucial in determining the trajectory of this evolving public health emergency.

The WHO has declared the 2024 Mpox outbreak a Public Health Emergency of International Concern, mirroring the response in 2022. As of August 2024, over 17,000 cases have been reported across 50 countries, with the virus predominantly affecting men who have sex with men, though broader populations are now impacted. Despite the WHO's declaration of #mpox as a Public Health Emergency of International Concern, there has been a surprising lack of discussion on important scientific questions. Notably, the effectiveness of the currently available vaccines - JYNNEOS (MVA-BN) and ACAM2000 - against the newly emerged clade 1b of the mpox virus remains uncertain. Studies have shown that while JYNNEOS provides cross-protection against orthopoxviruses, including previous mpox clades, its efficacy against clade 1b is not yet well-established. Similarly, ACAM2000, an older vaccine with a different safety profile, has not been rigorously tested against this variant. Given the potential for antigenic drift and changes in virulence, the absence of a more aggressive push to revaccinate at-risk populations with either of these vaccines is concerning and warrants investigation. As a final consideration, it’s important to ask why have we long ceased routine vaccinations in certain Western nations, despite the continued presence of diseases like mpox or even polio in other regions? While cost-benefit analyses have traditionally informed these public health decisions, relying solely on such metrics may fall short in our increasingly interconnected world.

Like many of you, I am?deeply concerned by the rising number of cases of?#mpox in Africa. It has now been declared a public health emergency of both continental and international concern by Africa CDC and Tedros Adhanom Ghebreyesus at the World Health Organization. The seriousness of these outbreaks in more than a dozen African countries serves as a reminder of the?constant threat of disease outbreaks and the importance of preparedness. Now, for mpox, this includes the?availability and?accessibility?of?vaccines and treatments to protect and save lives. Both Africa CDC and WHO have said they are working closely with companies who have approved mpox vaccines, and it is critical that the international community works to ensure they can reach the people who need them as quickly as possible. Following the declaration of mpox as a PHEIC, we now need to focus on: -Disease surveillance to comprehensively understand the epidemiology of mpox and ensure the right diagnostics are in place -Regulatory approval and emergency authorization for mpox vaccines -Procurement strategies that ensure available vaccines can be distributed where they will have the most impact, and orders are placed as soon as possible to enable manufacturers to scale up production accordingly -Immunization and healthcare delivery strategies that will help curb the spread of the disease and reach those whose need for vaccines and treatments is most urgent. This requires constructive collaboration across the international community.?We must focus on what needs to be done to control this disease – across multiple outbreaks of different clades through different modes of transmission and with different levels of risk – and how we commit to doing it.

The World Health Organization and Africa CDC have declared the upsurge of mpox in the Democratic Republic of the Congo and neighboring African countries a Public Health Emergency of International Concern (PHEIC). The rapid spread of the new and more dangerous form of the virus, known as clade 1b, underscores the urgent need for coordinated global action.
