Did you know hallux limitus was first described in 1887 by Davies Colley, who named it hallux flexus due to the plantarflexed position of the hallux on the first metatarsal? I learned this today. A quick review of the literature and I see that there is no consensus on the etiology of HL/HR. Thanks to Douglas Pacaccio,DPM, FACFAS for sharing the article by Patel and Swords with me. Patel J, Swords M. Hallux Rigidus. [Updated 2023 Nov 22] Tell me if I’m wrong, but it seems pretty cut and dry to me. With the exception of traumatic injuries and women who wore heels for 20 years, your average patient with slow onset of 1st MTPJ pain/DJD has suffered from chronic functional malposition of the 1st ray. The met head is elevated relative to the toe during heel off, causing dorsal joint impingement and wear and periarticular osteophyte formation. This begins as a flexible deformity and progresses to a stiff and rigid toe. In cases where there is still salvageable motion, my go to procedure has been a distal 1st met osteotomy with plantarflexion and shortening if needed. There is not always a need to fuse an arthritic joint. It works and my patients are thrilled to avoid a fusion. So LinkedIn, is the etiology structural or functional?
