Rotator cuff repair (#ARCR) is a common procedure for shoulder injuries, but retear rates remain a significant concern, particularly in older adults. Recent research has identified preoperative nutritional status as a key factor influencing surgical outcomes. The study, titled “Preoperative Nutrition Impacts Retear Rate After Arthroscopic Rotator Cuff Repair,” conducted by Dr. Hitoshi Shitara and colleagues at the Department of Orthopaedic Surgery, Gunma University Graduate School of Medicine, Japan, examines the relationship between nutrition and healing. Using the Geriatric Nutritional Risk Index (#GNRI) as a measure, the researchers found: ?Patients with a GNRI ≥109 had significantly lower retear rates. ?Patients with a GNRI <103 were 5.6 times more likely to experience a retear. ?65.7% of patients were found to have suboptimal nutritional status (GNRI <109) prior to surgery. These findings highlight that addressing malnutrition before ARCR is a critical opportunity to improve outcomes. With 67% of patients being sub-optimally nourished, this presents a significant, modifiable factor to enhance recovery and reduce complications. The study is now available as an Open Access publication in The Journal of Bone and Joint Surgery, Inc. This past week, the team at Xcelerated Recovery? had the immense honor of participating in the 2024 NYU Langone Health Orthopedics Annual Sir Robert Jones Meeting, a truly memorable event marking the 30th anniversary of Dr. Joseph Zuckerman, M.D. as Chair of the Department of Orthopedic Surgery. With over 300 attendees, this celebration showcased the strength, resilience, and enduring legacy of the NYU orthopedic community. A highlight of the event was Dr. Zuckerman’s keynote address on resilience, a topic that resonates deeply with our mission at Xcelerated Recovery. His reflections underscored the importance of perseverance, adaptability, and collaboration in shaping both professional excellence and personal growth. While we celebrated with those who were in attendance, it’s also important to acknowledge those who could not be there but whose contributions have been integral to NYU’s legacy. Together, we continue to honor and build upon the incredible foundation they’ve laid for future generations. The program was enriched by Dr. Michael Alaia leadership as Alumni Association President and Nicole Stevens’ vision as Program Chair, who curated an exceptional educational experience. The establishment of the Joseph D. Zuckerman, M.D. Educational Fund, which raised over $700,000, was a testament to the profound impact of Dr. Zuckerman’s leadership and the shared dedication to advancing orthopedic education and research. Come to Dallas 7 from 12:30-1:30 today (Friday 11/8/24) to hear Jameil ‘Jay’ Pendleton?, Brian Fuller, Cory Calendine, MD, and myself discuss perioperative nutrition. Two things tuned me into periop nutrition: my burn rotation as an intern where the surgeon had good outcomes but would delay skin grafts until the patients had prealbumin > 20, and research showing that patients with a BMI > 40 who were not malnourished actually had good outcomes. To refresh my understanding of nutrition I found an excellent article in EFORT Open Reviews (love the whole concept of this journal with its international insights and the fact that it is open access so nothing is behind a paywall). Kudos to the authors. https://lnkd.in/gVY3_beC Some insight as to how big of a problem it is: - Malnutrition is the strongest risk factor for poor postop outcomes in TJA PJI, aseptic loosening, periprosthetic fracture, dislocation, anemia, prolonged LOS, cost, mortality - No consensus exists regarding the methods to screen and diagnose malnutrition - The Joint Commission (boo, hiss) suggests preop nutrition screen should be performed within 24 hours of admission for surgical patients - Epidemiology: 15-60% prevalence of malnutrition in general adult population, 8.5-50% in TJA population - Clinical signs and symptoms may be subtle and often undetected until there has been a severe progression of malnutrition - Serologic assessment is not part of ASPEN (American Society for Parenteral and Enteral Nutrition) - Some thresholds recommended by the authors: serum albumin < 3.5, prealbumin <10-15 (should be < 20 in the eyes of #southgreat), transferrin < 200, vitamin D, zinc They only discussed prealbumin for a short paragraph which is a shortcoming. As mentioned in the first paragraph, I have used a threshold of > 20 for elective surgery with excellent outcomes in my practice (5 infections in 2800 cases). Prealbumin has a half-life of 2-3 days so it is easier to see changes in the short term (albumin half-life is 14-20 days). Management is only discussed for two paragraphs. There appears to be some inconsistencies in what I have heard as to exactly how much protein patients should be taking in the perioperative catabolic period. This article mentions 1 g/kg/d but I have seen numbers up to 2 g/kg/d. This is pretty much impossible to keep up for weeks after surgery so nutritional supplementation comes into mind. What difference (if any) exists between over-the-counter protein supplements and some of the branded nutriceuticals (or medicine as food)? A final question: How many companies would love to provide you with a solution that can easily decrease infection, revisions, decrease length of stay/cost, and even reduce mortality?
