For those who have already enrolled, you can access the new Patient Portal here: asap.ema.md.

We are just one week away from upgrading our Patient Portal to Modernizing Medicine (ModMed)! This update is designed to enhance your patient experience, and we are eager to share it with you. If you are a current patient with an upcoming appointment, please be on the lookout for an email this week with registration details for the new Patient Portal. This email will include a link to enroll as a patient with Associated Surgeons and Physicians, LLC. Please note that this link will expire after 72 hours. If your link expires, call us at 260-432-4400 for assistance. You will need to register for your Patient Portal using a web browser. We recommend Chrome or Firefox for the best experience. After you register for the Patient Portal using a web broswer, you will be able to log in with the ModMed App. If you expect to receive a registration email but do not find one by June 4, please check your spam and junk folders. We aim to introduce this new Patient Portal with minimal disruption. If you encounter any issues, do not hesitate to contact us at 260-432-4400.
