Today, WNYICC vaults into a new era of community support. Governor Kathy Hochul has chosen us as one of nine entities to lead the state’s Social Care Network (SCN) via its NYHER 1115 Waiver. ? As a result, WNYICC will receive up to $36.9 million in funding over the next three years to further support community-based organizations and develop our SCN in the state’s Western Region, which includes Cattaraugus, Chautauqua, Erie and Niagara counties. As a Community Care Hub, we’re dedicated to changing the way healthcare is provided. With a growing network of 70-plus social service organizations, our members produce better health outcomes and quality of life for residents by providing comprehensive, cost-effective, community-based integrated care. We look forward to partnering with community-based organizations, health plans and healthcare providers to close gaps in care, improve patient compliance with provider recommendations and enhance our region’s health.
