An American Heart Association study interviewed patients and clinicians in Accountable Care Organizations and found: ? Better patient care and outcomes ? Improved experience for clinicians ? Smarter technology and performance tracking “We see these life-changing impacts up close every day," says Wellvana CEO Kyle Wailes. "So it’s especially rewarding when others affirm that accountable care can be a rising tide where everybody wins.”

It’s not value-based care if the patient doesn’t win first. And you don’t just have to take my word for it. The American Heart Association is out with a fresh study that offers some independent data on the clinical benefits of participating in an Accountable Care Organization. It’s a small study, but it confirms much of what we see at Wellvana. ???In these survey-style interviews, patients “overwhelmingly” reported better care than they received under fee-for-service medicine. They noted stronger ties to their primary care provider, better care coordination and a team that included care managers and social workers. Many received help with transportation, food, housing and financial assistance. Those are the basics for any high-performing ACO. ?????It wasn’t just patients singing praises. Physicians and other clinicians also agreed that being part of an ACO enabled them to care for complex patients in a more holistic way. Because when you’re on the hook for all the costs surrounding a patient’s care, multidisciplinary teams become a must. And what’s good for the patient is also good for their providers. ???One more thing. Practices benefited from the technology support from an ACO. Primary care providers are often flying blind related to what a patient is doing outside of their office. But once they know a patient has been to the ER too many times in the past year, often for avoidable reasons, they can do something about it. We see these life-changing impacts up close every day. So it’s especially rewarding when others affirm that accountable care can be a rising tide where everybody wins.

Wellvana is in the business of accountability. Providers and patients trust us, and we deliver, unlocking the promise of value-based care. We’re in their neighborhoods and saved in their phones. Medicare Advantage payors are also accountable for costs and healthy outcomes, but there’s only so much they can do to influence behavior change with providers and patients. So many decisions are made behind the door of an exam room between the patient and their doctor. Effective value-based care requires an independent, impartial accountability partner with a stake in everyone’s success. Wellvana strengthens payor-provider collaborations with a focus on prevention. Our technology, focus on clinical measures and concierge-level care help improve an MA plan’s Star performance, and in turn, enhance the patient experience. ?? Before joining Wellvana as Senior Vice President of Medical Cost and Clinical Strategy, Rajul Gandhi, PharmD, MBA worked as a clinical pharmacist. He would get calls from geriatricians who almost always had the same request: Can you get this patient off at least one of their meds?? ? The National Institutes of Health says the regular use of five medications counts as polypharmacy. But it’s very common for patients with multiple chronic conditions to have a dozen or more meds. Some become unnecessary. Others turn out to be doing more harm than good. ? ?? In this article, Rajul explains a “Triple-A Antidote” to ensure tales of polypharmacy don’t turn into horror stories. Value-based care is all about streamlining care, including their medications. #valuebasedcare #primarycare #pharma #pharmacy https://lnkd.in/eVgjYCNx

?? For the next 53 days, seniors are making big decisions about their healthcare. It's all-skate. The Annual Open Enrollment Period allows anyone 65 and over to start a new Part D prescription plan, Medigap or Medicare Advantage plan. They can also switch MA plans or return to Original Medicare. But most don’t. A KFF analysis published this year found 7 in 10 Medicare beneficiaries did not compare their coverage, even though benefits and networks change each year. (link in comments) ?? It can seem like a tidal wave of information. It's complicated, which is why we’ve boiled it down in our 2024 Medicare Review, fresh with checklists and room for notes. (At least make sure the doctors you’ve seen in 2024 will honor your plan in 2025.) Share with (or tag) anyone who is approaching or beyond their Medicare birthday. As a value-based care enabler, Wellvana supports patients with all types of coverage, and those patients never see a bill for our help between appointments. But we know how important the right plan or combination of plans can be. Just like taking a 30-minute walk, meditating, scheduling annual screenings and your wellness visit, it’s good for your health – and your wallet – to take a yearly look at your coverage.

?? On Primary Care Week, we salute the heroes of value-based care — Primary Care physicians and health professionals. ? ? ?? Think about this: Primary Care benefits everyone, everywhere. And the more you visit your PCP and engage with their team, the lower your total cost of care will be in the long run.? According to a 2022 NIH study, the effect of primary care visits on total patient care cost?is even more stark for patients with multiple chronic conditions. (Link to study in comments) ? ? ?? Consider doing more than saluting your primary care team this week. Schedule time to see them, too.? Thanks to our Chief Medical Officer, Matt Rosenberg, MD who offers clinical leadership to hundreds of Wellvana’s partners who seek a better way to deliver care through value-based models.?And we’re forever grateful to our physician co-founder Saleh Jaafar, MD for making Wellvana what it is today.? ? Thanks also to the advocates for primary care in Washington, especially Accountable for Health, Primary Care Collaborative, America's Physician Groups and others. ? The way to Wellvana is in the hands of our skilled primary care professionals. https://lnkd.in/gp2JNRsQ #primarycare #valuedbasedcare #preventivecare

A patient in Georgia had a life-changing summer thanks to Foundational Care support from Ashley H., RN. This patient was in desperate need of inspiration. At 90 years old, she wasn’t sure it was even worth the effort to focus on her diabetes.? Her blood sugar was out of control, and she’d stopped taking her insulin. Plus, her blood pressure was in the danger zone. “Why waste my time?” she asked Ashley when they started working together in July.? In short order, this patient began to see that she could always improve her quality of life.? After a couple of months talking about diet and medication, this patient was doing so well that her blood pressure was barely in the hypertensive range. And her blood sugar was down by two-thirds.??? As a result, this patient hasn’t been to the hospital one time.? By the numbers, the support between in-office appointments from Foundational Care results in 14% lower ED visits and a 15% reduction in their costs.? Thank you to Ashley and the work of all our Foundational Care nurses. The service is vital to bringing more value to patients and their doctors. https://lnkd.in/g7Byi4pi??#valuebasedcare #primarycare #nurses

?? He’s worn so many hats since making his way to Wellvana in 2021. Here’s a new one for Readus Smith — President for National Partnerships U.S.? An experienced healthcare executive and entrepreneur with a passion for bringing world-class care to underserved communities.? ?? A relentless optimist who won’t rest in pursuit of a worthy goal.? ?? A charismatic leader who delivers on the hype. While value-based care is his vocation, his calling is as a lay leader in the Church of God in Christ. The appointment means he oversees the wellbeing of millions who are members of local congregations in all 50 states. And now his vocation and calling will come together.? He’s ideally suited to help the 5,000 doctors who are COGIC members - and thousands more clinicians in other faith communities - to transition from volume-based fee-for-service to patient-centered value-based care and bring new benefits to their communities.? It’s a novel partnership, but we hope it is the first of many as we spread the gospel about the Wellvana Interfaith Network.??(More details in comments). When Nelsabeth Gonzalez completed medical school in the Dominican Republic, she worked for three years in primary care then moved to the United States where she fell in love with value-based care. ? Her initial plan was to begin a residency in order to practice medicine in the U.S., so she started working as a medical assistant. Soon, she found herself tapping into her primary care background from the D.R.? ? Because she had the clinical fundamentals, Nelsabeth was able to help Accountable Care Organizations navigate risk adjustment and clinical documentation accuracy. This is how Medicare knows what kind of resources each patient will require each year. Without accurate risk adjustment, value-based care can’t work. During Hispanic Heritage Month, we honor Nelsabeth’s way to Wellvana and her contributions to transforming the U.S. healthcare system.
