We’ve posted a few different use cases in recent weeks, highlighting the adaptability of ‘allco’ in serving the social needs of community members. ? We’re pleased to announce our latest integration supports community-based pharmacies in New York state. ? Led by the University at Buffalo Clinical and Translational Science Institute, the project will involve implementing ‘allco’ at pharmacies that serve the local needs of their communities. ? Pharmacy staff will use ‘allco’ to conduct assessments of their clients, determining whether they have health or social needs and then using the platform to refer them to community-based care. ‘Allco’ will enable the documentation of this activity to assess its viability and the potential for a larger rollout in subsequent phases. ? The use of ‘allco’ to create a referral network and document referrals has led to extremely high ‘closed-loop referral rates’ with existing clients, validating our argument that data-informed processes can dramatically improve community-based care. We are excited to begin the second phase of allco support for the innovative Monmouth ACTS program, a community-wide rollout with human services at its beating heart. The first phase of this countywide program established a state-of-the-art electronic referral system between NJ 211 Partnership, Monmouth County and the nonprofit social care agencies that work with the county. That involved hundreds of referrals each month from the 211 to those agencies through ‘allco,’ and documentation that those referrals were completed. Before Monmouth ACTS, there was no mechanism to track the volume and success rate of those referrals. We know that sounds like jargon. We like to think of it like this: every closed-loop referral means a human being got the care they were seeking. The second phase expands beyond referrals so that all 60-plus of those agencies will adopt ‘allco’ for full case management, record-keeping and reporting capabilities. This means that Monmouth County, New Jersey, will have a community information exchange that ties all of the different human services stakeholders into a collective system. It’s a point of pride and gratitude for our team to be part of these projects with partners who believe there is a better way to plan, track and deliver human services. The community information exchange project in Cook County, Illinois, is leading to broader recognition of our mission to create shared data between healthcare and social care agencies. But we’re after something bigger than data integration and software. ‘allco’ is about leveraging the social determinants of health to create crucial context for community members. We want a doctor to know if a patient has been to a homeless shelter. We want a soup kitchen to be able to make a seamless referral to a behavioral health clinic. That means wellconnected needs to think holistically about bridging the gap between healthcare and social care. From incentive models to technology structures to business culture, we’re writing the playbook on how to get stakeholders on the same page so they can use the data, not just create it. The Cook County version of this initiative recently began in concert with Brian Handspicker, a national expert in interoperability standards, who will work with us to drive the conversation on consent models in Chicago. Together with project partners 4medica - One Patient...One Record and New Jersey Innovation Institute, we’re creating a turn-key implementation structure that will be applicable to CIEs across the U.S.

Our work in Cook County, Illinois, is starting to get into the weeds as we prepare to connect ‘allco,’ our networked enterprise platform, into 4medica - One Patient...One Record's health information exchange. This is the operational side of a big goal: creating shared data between doctor’s offices and nonprofit agencies that allow both sides to use social determinants of health to create better care for individuals. wellconnected is cruising into 2025 on the back an exciting win: a groundbreaking initiative in Chicago to create a community information exchange that integrates both healthcare and social records (along with partners Illinois Public Health Institute (IPHI), 4medica - One Patient...One Record and New Jersey Innovation Institute). The announcement capped a year of tremendous growth that saw us expand into three new states as our vision for a transparent, collaborative social safety net begins to take hold. This is a tech-enabled vision of the future, for sure, but software is only part of the story. We’re introducing a new operating process for entire regional social care communities, taking disconnected oases of care and support and turning them into a thriving interconnected rainforest.
