    This year’s Rural Health Day theme is “Advancing Maternal Health in Rural Communities,” an opportunity to celebrate initiatives that improve health outcomes for some of our most vulnerable members. In rural Fresno County, CA, Health Net, a Centene company, engaged a community advisory workgroup to better understand Salvadorian cultural practices around postpartum care. Through education and tailored communication strategies, Health Net worked with providers to create culturally sensitive approaches to care. As a result, postpartum visits increased from 50% to 82% over a two year period. #RuralHealthDay #RuralHealth #PowerofRural

    Congratulations to Mike Maynard of the Hillcrest-Bruce Mission for being one of WellCare of Kentucky's 2024 Regional Community Health Champions! WellCare of Kentucky's Community Health Champions Awards honor individuals and organizations throughout the Commonwealth that play a pivotal role in removing barriers, improving health outcomes and supporting community well-being. Maynard founded the Hillcrest-Bruce Mission in 1971 with a vision to alleviate the suffering caused by generational poverty. His commitment to building a community that is unconditionally loved, supported, encouraged and equipped to rise out of generational poverty is evident in every aspect of his work. ? Under Maynard’s guidance, the Mission has grown its reach and impact by expanding its facilities and programs to also help meet clients’ spiritual, physical, emotional, educational and social needs. ? Through his leadership, the Mission has helped hundreds of people in Ashland, Ky. find homes, vehicles, jobs and even earn college degrees. Recently, the Hillcrest-Bruce Mission transformed a food pantry into a shopping experience where families could choose what they need. “I see our Community Health Champions as examples of the best of Kentucky,” said WellCare of Kentucky Plan President & CEO Corey Ewing. “Each of these individuals and organizations has worked to ensure that Kentuckians are physically, emotionally and mentally healthy – and they did it in deeply personal ways, giving their time, talent and heart in the process.” ?     Congratulations to Danelle Slack, vice president of case management at Mayfield-Graves County Long Term Recovery Group (LTRG), our WellCare of Kentucky Community Health Champions overall award winner! WellCare of Kentucky's Community Health Champions Awards honor individuals and organizations throughout the Commonwealth that play a pivotal role in removing barriers, improving outcomes and supporting community well-being. In December 2021 when the EF-4 tornado ripped through western Kentucky, with over 845 cases in Graves County, Slack made an impact on each of those survivors. She worked with national, regional and local organizations to help develop the Mayfield-Graves County LTRG and drilled down into the case management process to make sure that every survivor had an equitable recovery. ? By committing herself to the work of recovering survivors, Slack has led the Mayfield-Graves County LTRG case management by helping 168 families reach their recovery goals. She has helped survivors with housing, aided in the collective effort by many building partners in Graves County to get more than 90 families into new homes. In the wake of the tornado, for months, Slack sorted clothes in her garage that served as a makeshift warehouse. She continues to go above and beyond by making sure that all survivors placed in their homes receive a welcome home basket. “I see our Community Health Champions as examples of the best of Kentucky,” said WellCare of Kentucky Plan President & CEO Corey Ewing. “Each of these individuals and organizations has worked to ensure that Kentuckians are physically, emotionally and mentally healthy – and they did it in deeply personal ways, giving their time, talent and heart in the process.” This year’s 12 winners were selected from over 250 nominations stateiwde.     Congratulations to Mandy Heacock, the family resource coordinator at Bondurant Middle School’s Youth Services Center, for being one of WellCare of Kentucky's 2024 Regional Community Health Champions! WellCare of Kentucky's Community Health Champions Awards honor individuals and organizations throughout the Commonwealth that play a pivotal role in removing barriers, improving health outcomes and supporting community well-being. Determined to eliminate financial barriers for students, Heacock created a spot where students can go and “shop” at no cost for clothes. She also created a sports equipment closet for students who can’t afford the gear necessary to play certain sports. Every August, Heacock leads a shoe giveaway in Franklin County. Because of her effort and leadership, students have received more than 2,000 new pairs of shoes during the last six years. Heacock isn’t just focused on the things students will need for school. Dedicated to improving the lives of veterans living in southern Kentucky, Allen started Southern Kentucky Patriots (SOKY) in 2019. Its goal is to reduce hardships for veterans, low income and disabled individuals; however, Allen’s passion for helping doesn’t stop there. He also piloted a food backpack program that ensures no child in Warren County goes hungry over the weekend.
