    Brain disorders like #Parkinson’s Disease (PD) start to develop in patients much earlier than when their first clinical symptoms appear, and it's highly likely that treating patients at these early stages could slow or even stop their disease. Currently, however, there is no way to diagnose brain disorders at those pre-symptomatic stages, which poses a major challenge to society at large. David Walt’s team at the Wyss Institute at Harvard University and Brigham and Women's Hospital are advancing their "liquid biopsy" platform for detecting #biomarkers of diseases like PD in tiny extracellular vesicles released by all organs into blood in a technological tour-de-force. Their new study enables them to take an exclusive, ultra-sensitive peak at biomarkers that are inside EVs and not unspecifically attached to them – a first in this busy field. They were able to specifically home in on the PD-specific cargo protein??-synuclein in its phosphorylated form and demonstrated that it is significantly elevated inside of EVs in samples from PD patients.     The long-awaited results of the Exenatide-PD3 study have finally been presented. In this crucially important phase 3 study, 194 participants were randomised to receive either #exenatide (a drug from the #GLP1 agonist class that stems originally from the diabetes field, and which has been repurposed for use as a possible disease modifying treatment for people with #Parkinson disease) or placebo. To everyone's regret, there was no difference between the two study arms, neither for the primary outcome nor for any of the other outcome measures. This is very disappointing, particularly because two out of three earlier phase 2 studies with GLP-1 agonist (one also with exenatide) did point to a possible disease modifying effect. The research team and the sponsors are to be commended for sharing these findings with the trial participants first, before anybody else would hear. I think that most of us would agree that disappointing as this may be, this basically puts an end to the hopes that drugs from the GLP1 agonist class might become the first pharmacotherapeutic approach to slow down the progression of Parkinson's. The study was exceptionally well designed, and to my mind serves as a model for future studies that will test other pharmacological as well as nonpharmacological strategies (including our own STEPWISE and Slow-SPEED trials where we test the disease modifying effects of #exercise). https://lnkd.in/eazAmZVr Cure Parkinson's Simon Stott Parkinson's Foundation The Michael J.     Parkinson's disease (PD) is a progressive neurological disorder that affects movement, mental health, sleep, and may eventually develop dementia. While there is no cure, various therapies and medications can alleviate symptoms and improve quality of life. Family members and caregivers play a crucial role in assisting with daily activities and providing emotional support. Therapeutic interventions include medication, rehabilitation, speech therapy, and dance therapy. Despite the challenges posed by Parkinson's disease, effective therapies and supportive care can help individuals manage symptoms and maintain a fulfilling life. P.S.
