    40% of acute care visits for Medicaid beneficiaries are avoidable due to gaps in access to timely care — this is 3x the rate seen in patients with Medicare or commercial insurance coverage. As federal budget cuts loom, the need for innovative, cost-effective care models that generate real savings for our healthcare system while keeping patients healthier has never been more urgent. Recently, Waymark cofounders Rajaie Batniji MD DPhil, Sanjay Basu, and Afia Asamoah joined a16z general partner, Vineeta Agarwala, MD PhD, to share more about what Waymark is building. The founders shared how Waymark uses novel “rising risk” machine learning models to identify Medicaid patients who could benefit from timely, community-based interventions – these interventions have reduced avoidable ER and hospital visits by 23%, while also improving care quality by 11 percentage points (as measured by HEDIS metrics) and helping patients meet their own goals.     This month’s edition of ?Transforming Medicaid ?, a monthly social series where we dive into the data, stories, and evidence base around improving Medicaid, focuses on one of the biggest challenges facing Medicaid today: access to high-quality behavioral healthcare. Historically, Medicaid has faced a barrage of ongoing challenges in providing behavioral health support to beneficiaries, including: ?? Severe workforce shortages, particularly among therapists and psychiatrists, leading to extended wait times for patients before receiving care. ?? Low payment rates: Medicaid often underpays behavioral health providers compared to other payers, discouraging participation. ?? Numerous social risk factors can prevent Medicaid patients from accessing behavioral health support when they need it. In many communities, Certified Community Behavioral Health Clinics (CCBHCs) have emerged in response to these challenges. Funded through Medicaid demonstration programs and serving 3 million patients nationally, CCBHCs make comprehensive behavioral health services available under one roof, including crisis services, outpatient treatment, rehabilitation services, and peer and family support. As of 2024, 98% of CCBHCs are engaged with criminal justice agencies in at least one partnership aiming to lower recidivism and/or incarceration rates, and more than 80% of CCBHCs offer on-site services in youth-serving settings. To continue to scale the community impact of these clinics, four states have increased their funding, and many more states have received grants from the Substance Abuse and Mental Health Services Administration (SAMHSA) to augment their funding. They also highlight findings from our 2024 study published in NEJM Catalyst, which underscores how our early interventions helped over 2,000 PCPs and two Medicaid health plans reduce unnecessary ED visits by 20% in our first year of service.
