??Clinical Case with Mithilesh Das, MD, FACC, FHRS, MBA, Indiana University Health:?Male patient in his 40s with a recent episode of syncope.?The patient presented to Dr. Das for a second opinion following an MRI that was negative for substrate and a diagnosis requiring a device. ?? Patient was induced VT with premature pacing using triples. The VT was mapped using outflow tract library and received hotspots on the RVOT lateral pulmonary cusp. ?? Dr. Das ablated within this area. After ablation, pacing with triples was done again, which induced a new VT. The VT was analyzed, showing hotspots on the RVOT anterior-septal. Pace match was strong and Dr. Das ablated in that location. He also saw how the vMap hotspot aligned with that area. VT was induced again and had a 90% match to the previous VT. Dr. Das applied additional ablation to the previous site where the vMap hotspot was located. The patient was paced with triples down to 200 and was non-inducible. ?? This case demonstrates how vMap can positively impact outcomes and help prevent the patient from receiving unnecessary additional procedures. #epeeps #cardiology #EPLab #patientcare #vMap
