The Medicare Competitive Bidding Program was implemented in 2011 to reduce durable medical equipment prices. Legislation introduced in 2024 aims to remove supplemental oxygen from the Program due to concerns that decreases in oxygen prescribing are due to decreased supply and limited oxygen access for patients with chronic lung diseases -- driven by lower prices set by the Program. Conversely, low-value prescribing of oxygen is prevalent in practice, and decreased oxygen prescription rates may not have necessarily caused harm. What is the association between the Program and patient use, outcomes, or spending? I had the priveledge of attending the #SOCRA2024 annual conference and sharing our experience of building a comprehensive, centralized, #diabetes screening program and designing it in a way that also captures randomized #research data to continue studying and optimizing this effort at our county hospital! In the United States, health inequity is a critical issue that has not been systematically addressed by broad policy reforms. To improve equity, policymakers must specifically target and resource ambulatory “safety-net” organizations serving historically marginalized populations. Not doing so risks reforms that fail to address these populations’ needs and close equity gaps. Currently, there is no consensus definition of the ambulatory safety net, limiting policymakers’ ability to support it.
