Yesterday’s #ClinicalChallenge question was: Xanomeline-trospium, a new antipsychotic agent for schizophrenia, has a lower risk of which adverse effects compared with other agents? A. Urinary retention B. Extrapyramidal side effects C. Pronged QTc interval D. B and C ?? The correct answer is: D. Xanomeline-trospium is the first antipsychotic agent to exclusively target muscarinic receptors. Adverse effects include urinary retention, increased heart rate, decreased gastric motility, nausea, vomiting, constipation, and hypertension. However, it is associated with fewer side effects (e.g., extrapyramidal symptoms) than most commonly used dopamine-blocking agents and has minimal effect on the QTc interval. We're thrilled to announce that Lisa Cosmi and Nicola Sciannandrone from the Italian Wolters Kluwer Health Clinical Effectiveness team have won the Hackathon organized by Italy's Regione Piemonte and Azienda Zero Piemonte on Healthcare Logistics in the "Services to General Practitioners and Pediatricians" category. It also complements the innovative logistics models already implemented by the Regional Health Authorities.

In a randomized trial in patients with gestational diabetes mellitus comparing a sequential strategy of beginning metformin therapy and adding glyburide if glucose targets were not met versus a strategy of insulin alone, the oral antihyperglycemic strategy resulted in higher rates of large-for-gestational-age infants and maternal hypoglycemia.
