A new meta-analysis including over 17,000 college athletes and military recruits reported that 31 percent had iron deficiency (ferritin <30 ng/mL). Female athletes/recruits were more likely to be affected than male athletes/recruits; prevalence was similar in those <18 versus ≥18 years of age. These observations confirm that high-intensity physical training is a significant risk factor for iron deficiency and support a role for screening in this population. Hypovolemic phlebotomy?before liver transection involves removing 7 to 10 mL/kg of whole blood from the patient without replacement with intravenous fluid before transection and then returning all of the blood after transection. In a randomized trial in over 440 patients, hypovolemic phlebotomy reduced the 30-day transfusion rate compared with usual care (16 versus 8 percent), without significantly affecting severe or overall complication rates.
