Our geography gives us a secondary care catchment population of 450,000 with a wider peninsula population of almost 2,000,000 people who can access our specialist services. The population is characterised by its diversity – the rural and the urban, the wealthy and pockets of deprivation, and wide variance in health and life expectancy. We work within a network of other hospitals to offer a range of specialist services such as kidney transplant, cardiothoracic surgery and neonatal intensive care and high risk obstetrics. Our Discharge Lounge helps us to better support patients who are ready to be discharged, making things more convenient and comfortable for them. Situated at the rear of the Derriford Site with an outdoor area, the Discharge Lounge offers a restful environment for patients who are waiting to go home. There’s comfortable seating and support available. Staffed by registered nurses, the Discharge Lounge team care for patients as they wait to be discharged, checking that they receive their medication, ordering transport and contacting their family, friends or carer. Your checklist for discharge to help you keep well this winter: ? Will someone pick you up or meet you at home? ? Are your ongoing care arrangements in place? ? Do you have suitable outdoor clothing and shoes to wear? ? Do you have any medical certificates you may need? ? Do you have your hospital discharge letter for your GP? ? Do you have your medication ready to take home? ? Do you understand how and when to take your medication? ? Do you have your house keys and personal belongings? #Winter #Healthcare #NHS

Sean Wylie, a Healthcare Science Practitioner Apprentice at University Hospitals Plymouth NHS Trust (UHP), has won the coveted UHP Apprentice of the Year title for 2024. UHP hosted a week-long programme of events to celebrate National Apprenticeship Week which took place from 10 to 16 February. One of the highlights of the week was the crowning of this year’s UHP Apprentice of Year. Sean Wylie, who is currently studying at the University of West England (UWE) for a level 6 apprenticeship as a Healthcare Science Practitioner based in the Radiotherapy Physics department, was presented with the award alongside a surprise from the Radiophysics department. The Apprenticeships team, who are part of UHP’s Learning and Organisational Development team, celebrate valued apprentices each month, giving out Apprentice of the Month awards. The team then collate the winners from across the year and invite Trust colleagues to vote for the coveted Apprentice of the Year award. Speaking about his success this year, Sean said: “I am really thankful for winning the award and it was a big surprise to see all of the team there clapping. I am thoroughly enjoying the course, gaining so much experience from it, and the team are really helping me to get the most from the opportunity.” UHP offers an extensive range of apprenticeships across the organisation, in both clinical and non-clinical disciplines and regularly celebrate the success of our apprentices. Sean’s win was part of a week-long programme of events focussing on apprenticeships from level 2 to level 7. Events throughout the week included a visit from the City College Plymouth team for a session in the Career’s Hub, which is a jointly run venture from UHP and City College Plymouth, employer encounter events in the community, and certificate presentations for new qualified colleagues. University Hospitals Plymouth NHS Trust (UHP) has been rated the most improved Trust in England in relation to the 4-hour target despite increased demand for the Emergency Department. In data shared by NHS England, UHP is leading the trusts who have made the most improvement regarding the 4-hour waiting time target for patients attending our Urgent and Emergency Care services and Minor Injury Units during the 2024/25 year. The 4-hour target for 2024/25 is to see and treat 78% of patients within 4 hours of them arriving. In 2023/24 UHP was seeing an average of 54.9% of patients within 4 hours, but with the recent improvements this has risen by almost 10% to 64.8% in the 2024/25 financial year to date.? UHP is also among the top 10 trusts in the country for improvements in the number of patients waiting more than 12 hours in the Emergency Department. In 2023/24, 18% of patients attending as emergencies would have waited more than 12 hours, however, in the 2024/25 financial year to date this has decreased to 16% of patients. These improvements have been made despite demand for emergency services at UHP increasing by 6.1% in 2024/25, equating to an additional 4,646 attendances. Meeting this additional demand and improving the experience for the almost 300 patients attending the Emergency Department each day has been possible due to the commitment and dedication of colleagues who are working on the Trust’s ‘One Plan’ – a programme which is driving improvements in the quality of urgent and emergency care for patients.
