The CSB is an independent federal agency charged with investigating chemical incidents. Headquartered in Washington, DC, the agency's board members are appointed by the President and confirmed by the Senate. The CSB conducts root cause investigations of chemical incidents at fixed facilities. Root causes are usually deficiencies in safety management systems, but can be any factor that would have prevented the accident if that factor had not occurred. The agency does not issue fines or citations, but does make recommendations to plants, regulatory agencies such as the Occupational Safety and Health Administration (OSHA) and the Environmental Protection Agency (EPA), industry organizations, and labor groups. Congress designed the CSB to be non-regulatory and independent of other agencies so that its investigations might, where appropriate, review the effectiveness of regulations and regulatory enforcement.

Volume 2?covers 25 serious chemical incidents in 14 states: Arkansas, Alabama, California, Colorado, Connecticut, Illinois,?Louisiana, Mississippi, Nebraska,?Nevada, New Mexico, Ohio, Oklahoma and Texas.?These serious incidents resulted in 7 fatalities, 23 serious injuries, and approximately $1 billion in property damage.

The U.S. Chemical Safety Board and Hazard Investigation Board (CSB)?released an update?into its ongoing investigation into the fatal November 12, 2024, explosion at the Givaudan Sense Colour facility in Louisville, Kentucky.?The incident killed two employees and seriously injured three others. The explosion caused significant property damage to the facility and nearby homes and businesses. Debris was ejected outside the facility's fence line, flying up to 400 feet into the nearby neighborhood, and local authorities issued a shelter-in-place order for a one-mile radius around the facility.

The U.S. Chemical Safety and Hazard Investigation Board (CSB) announced that it is launching a new initiative to give the public more details about the serious chemical incidents that have been reported to the CSB since the agency’s Accidental Release Reporting Rule (ARRR) went into effect in March 2020.?The CSB will be compiling summaries of reported incidents and making them available to the public on the CSB’s website on a regular basis. The CSB released Volume One of the Incident Reports on January 14, 2025.
