We would not be here without the compassion and unwavering commitment that our social workers show our residents and their families every day that truly make Troupe Health #AgeInPlace program unique & special. ?? ?? Sarah Coleman, Lead Social Worker, has been with Troupe since March 2024. As our first clinical hire, Sarah has paved the way for what our patient journey looks and feels like. She has spent countless hours enrolling our first patients, creating workflows and processes, and developing relationships with partner communities like #CaringHearts. ?? Hilka Rivera-Cruz is our Orland-based social worker hired in January 2025. She has brought tremendous expertise to our team in the area of hospice and care planning, as well as the ability for us to scale and grow with new community partners. Hilka has already made a lasting impact here at Troupe in her first 90 days. ?? Laura Soliman is our social worker in Tampa who provides behavioral health services for our partner communities within that market. Her experience as a former in-home social worker under a major healthcare payer makes her an invaluable asset to delivering out services in-person to residents within their 'home' or community. Thank you to these amazing women who are such an essential part to our Troupe Health team and provide highly needed care to our residents and their families. ??

She covered the different types of dementia, ?? early signs, risk factors, and the impact on individuals and caregivers. Beyond education, her talk created a space for families to seek guidance. ?? Hilka shared: "I had many coming to me after saying thank you ?? and feeling informed on how to connect with their loved ones. One family asked about their loved one on hospice with dementia, wondering what to expect. I was able to provide that perspective—helping them navigate end-of-life decisions based on quality of life." ?? This conversation highlights the need for compassionate education and support in dementia care. ?? March is National Brain Injury Awareness Month ?? Brain injuries, including traumatic brain injuries (TBIs) and concussions, can have devastating effects—especially for older adults. Falls are the leading cause of TBIs in seniors, and the consequences can include cognitive decline, mobility issues, and increased health complications. According to NAMI (National Alliance on Mental Illness), brain injuries can also contribute to mental health challenges, such as depression, anxiety, and PTSD. Despite these risks, many TBIs go undiagnosed, leaving seniors vulnerable to worsening health outcomes. ?? How can we help?
