Trenton Health Team has been serving Trenton for more than a decade as a community healthcare collaborative dedicated to improving access to quality healthcare for city residents. The collaborative is an innovative partnership among St. Francis Medical Center, Capital Health, Henry J. Austin Health Center, and the City of Trenton Department of Health and Human Services. Over the years, our focus has expanded to meet the range of health concerns affecting our community. THT now operates with a broader understanding of what health and well-being — and health equity — mean for Trenton, as well as what activities and strategies will help address all the factors influencing a healthy city. Our shared Vision for a Healthy Trenton incorporates social determinants of health, defining a “healthy Trenton” that includes: * Healthy Environment and Housing * Vibrant Public Spaces * Growing Economy * Healthy Food * Improved Safety * Equity and Social Cohesion * Quality Healthcare To realize our Vision for a Healthy Trenton, THT has embraced a broader view of health and well-being in the community and is working with community and healthcare partners to advance that objective.

THT's GIS Analyst, Em Ostrowski, had the opportunity to present their research on the Mercer County Food Insecurity Index at the Esri User Conference in San Diego, California this week! ?? This conference brings experts from around the world together to share knowledge and innovations in geographic information systems. ???? Em's presentation highlighted the creation and application of an index to better understand and address food insecurity in a region. They say it was a fantastic experience to engage with fellow attendees and discuss the impact of our work! #EsriUC2024

Today, we celebrate #Juneteenth, which marked the end of slavery here in the U.S. It also serves as a powerful reminder we still have a long way to go in ongoing efforts towards equity. This makes us reflect on last year when our CEO, Greg Paulson, and Chief Community Care Officer, Cheryl Towns, were invited to Bank of America's Combined Leadership Development Program in Charlotte, NC. There, they had the opportunity to meet "The Grandmother of Juneteenth," and social activist, Opal Lee, who successfully advocated for Juneteenth to be recognized as a federal holiday.

Kim recently shifted from overseeing our harm reduction work to our maternal health projects and, will therefore, co-chair our Maternal Health Stakeholders group convenings. Kim has worked in educational settings from elementary to secondary school in a variety of special education programs before she transitioned into public health. As a mom of 2, she brings her lived experience in the maternal and reproductive health space to this role, which has made her passionate about improving experiences for birthing people, particularly Black birthing people, in our area! ?? #TrentonNJ #NewJersey #MaternalHealth
