Transition Professionals is a nonprofit organization formed to assist people released from jail or prison. Our primary goal is to reduce recidivism by providing case management, support, services, clothing, food and guidance to ex-offenders. Through Transition Professionals, ex-offenders can find help with: -Access to housing -Employment -Mentorship -Introduction to social services -Medical/Mental support -Referrals -Ancillary support Our mission is to offer a One Stop Center to assist ex-offenders as a gateway to services and support needed upon immediate re-entry into to the community.

Transition Professionals is hiring an LCSW for a one year per diem commitment. We work with people who are justice involved and may have been formerly incarcerated. Position is part time, 20 hours a week and per diem paying $40 to $50/hour depending on experience.
