??? This is a common question that we get asked in Hospice care. Let’s take a more in depth look at artificial means of nutrition and hydration in the Hospice setting (i.e. feeding tubes, and IV hydration) to give you a better understanding of the risk versus the benefit. Take into consideration that as we near end of life the body systems start to slow down, this includes the nutritional requirement. ????Artificial nutrition and hydration is a medical intervention that aims to provide benefits but is also associated with several side effects and complications. Decisions regarding its use should be guided by an objective evaluation of the potential benefits, the likely side effects and burdens, and the goals of the individual and their family concerning the treatment. ?????? Dr. Cheryl Arenella from the American Hospice Foundation states: “When artificial nutrition and hydration is more likely to be burdensome than helpful, it should be avoided or discontinued.? Nurturing can be expressed in more helpful ways, such as gentle presence, touch, talking with the person (regardless of his/her ability to respond), keeping the person’s lips and mouth moist, gently massaging the skin using lubricants, praying with the person, or playing favorite music selections.? These alternative ways of nurturing can be very powerful and moving for both the person with the life-threatening illness and his/her loved one.” Using these alternative methods at end of life promotes the most optimal outcome, prevents adverse side effects, and increases overall quality of life. People might feel numb or in shock and may find it difficult to believe that the loss has occurred. ???EXAMPLE: "This can't be happening," or "This isn’t real." 2?? Anger ?What It Is: As the reality of the loss sets in, individuals may feel anger. This anger can be directed at themselves, others, the situation, or even the person who has died. ???EXAMPLE: "Why is this happening?" or "This is unfair!" 3?? Bargaining ?What It Is: During this stage, people may try to negotiate or bargain in an attempt to reverse or lessen the loss. This can involve making deals with a higher power, or even making promises to themselves to change their behavior if only the loss can be undone. ???EXAMPLE: "If only I had done something differently," or "I would do anything to bring them back." 4?? Depression ?What It Is: This stage is characterized by deep sadness, despair, and withdrawal. Individuals may feel overwhelmed by the magnitude of the loss and experience a sense of hopelessness. ???EXAMPLE: "I miss them so much," or "What’s the point of going on?" 5?? Acceptance ?What It Is: Acceptance doesn’t mean being okay with the loss, but rather coming to terms with it and finding a way to move forward. It involves acknowledging the reality of the situation and beginning to adjust to life without the loved one. ???EXAMPLE: "I’m at peace with what happened," or "I can move on now." ?? Additional Considerations: ?? Not Linear: It’s important to remember that these stages are not experienced in a set order, and not everyone will go through all five stages.
